替罗非班在颅内动脉瘤栓塞术中急性血栓形成的应用

Application of tirofiban in acute thrombosis during embolization for intracranial aneurysms

目的探讨替罗非班在颅内动脉瘤介入栓塞术中急性血栓形成的应用。方法回顾性分析8例颅内动脉瘤行介入栓塞治疗术中出现血栓形成的病例资料,均给予导管内推注替罗非班。结果血流复通7例,血流m TICI 3级,术后未出现相关临床并发症,CT复查未见出血及新发脑梗死;复通不理想1例,血流m TICI 2b级,术后对侧肢体偏瘫,肌力3级,CT复查示中央回新发脑梗死。结论替罗非班指引导管内推注治疗颅内动脉瘤介入栓塞术中脑血栓形成安全、有效。

Objective To evaluate the efficacy of tirofiban in acute thrombosis during embolization for intracranial aneurysms. Methods Clinical data of 8 patients with intraoperative acute thrombosis during embolization of intracranial aneurysms were analyzed retrospectively, and tirofiban was infused into guiding catheter in every patient. Results The vascular recanalization was achieved in 7 patients with mTICI grade 3, and no related complications occurred postoperatively, and no rebleeding or new cerebral infarction was found according to CT reexamination. While partial recanalization was achieved in 1 patient with mTICI grade 2b, and contralateral limb hemiplegia with myodynamia of grade 3 occurred after surgery, and cerebral infarction in the central gyrus was found by CT reexamination. Conclusion Infusion of tirofiban into guiding catheter is safe and effective for the treatment of thrombosis during embolization of intracranial aneurysms.
