成人血清25羟维生素D与免疫球蛋白E水平的相关性研究

Association of serum 25-hydroxyvitamin D with serum IgE levels in adults

目的探讨成人血清25羟维生素D[25（OH）D]水平与IgE水平之间的相关性。方法入组1 969名19岁以上的参与者进行研究，检测血清总IgE（TIgE）、粉尘螨敏感性IgE（DfSIgE）和血清25（OH）D。采集性别、年龄、BMI、吸烟史等资料。校正后线性回归分析25（OH）D与lgTIgE的相关性。结果1 969名受试者血清25（OH）D平均水平为（18.0±6.5） μg/L，哮喘、过敏性鼻炎和特应性皮炎的患病率分别为15.2%（299/1 969）、2.8%（63/1 969）和3.0%（68/1 969），但在有无过敏性鼻炎的患者中，血清25（OH）D水平差异无统计学意义。根据血清25（OH）D水平分组：Q1：〈13.4 μg/L；Q2：≥13.4～〈17.0 μg/L；Q3：≥17.0～≤21.7 μg/L；Q4：≥21.8 μg/L，通过Kruskal-Wallis检验，TIgE的中位数随着25（OH）D的升高而升高。Q1、Q2、Q3、Q4组中Df过敏的患病率分别为39.8%、33.2%、38.7%和43.1%，校准后的线性回归分析提示：25（OH）D水平与lgTIgE呈正相关（回归系数：0.011，95%CI 0.001～0.021）；男性粉尘螨过敏患者25（OH）D与lgTIgE水平明显正相关（回归系数：0.026，95%CI 0.010～0.043），其他组25（OH）D与lgTIgE无明显相关性。结论成人血清25（OH）D水平与TIgE水平呈正相关。

ObjectiveTo investigate the correlation between serum levels of 25 hydroxyvitamin D and immunoglobulin E（IgE） in adults. MethodsThere were 1 969 participants over the age of 19 were enrolled in this studied, serum total IgE, dermatophagoides farinae（Df）-specific IgE, and serum 25（OH）D levels were analyzed.Other variables included sex, age, body mass index, smoking history, and economic status were collected.Adjusted regression analysis was used to examine the independent association of 25（OH）D with serum IgE levels.ResultsThe average serum 25（OH） D level of 1 969 patients was （18.0±6.5） μg/L, the prevalence of asthma, allergic rhinitis, and atopic dermatitis were 15.2%（299/1 969）, 2.8%（63/1 969） and 3.0%（68/1 969） respectively, however, there was no significant difference in serum 25（OH） D levels in patients with allergic rhinitis.All subjects were divided into four groups according to the serum 25（OH）D level： Q1： less than and equal to 13.4 μg/L, Q2： more then and equal to 13.4 μg/L but less then and equal to 17.0 μg/L, Q3： more then and equal to 17.0 μg/L but less then and equal to 21.7 μg/L, and Q4： more than and equal to 21.8 μg/L.The median TIgE level increased with higher quartiles in the Kruskal-Wallis test.The prevalence of Df in Q1, Q2, Q3, Q4 group were 39.8%, 33.2%, 38.7% and 43.1%, respectively.Linear regression analysis indicated that 25（OH） D levels were positively correlated with lgTIgE （regression coefficient： 0.011, 95%CI 0.001-0.021）, 25（OH）D in male patients with dust mite allergy was positively correlated with lgTIgE（regression coefficient： 0.026, 95%CI 0.010-0.043）. There was no significant correlation between 25（OH）D and lgTIgE in the other groups.ConclusionAdult serum 25 （OH） D levels is positively correlated with total IgE levels.
