摘要
目的探讨氨基末端脑钠肽前体(NT-proBNP)在心源性和肺源性呼吸困难诊断中的意义。方法选取以呼吸困难为主要症状患者163人,分为心源性呼吸困难组85例和肺源性呼吸困难组78例,并选取健康体检者31例为对照组。采用电化学发光法检测三组NT-proBNP浓度,并进行统计学分析。结果心源性呼吸困难组血清NT-proBNP浓度为(218.24±126.99)pg/m L;肺源性呼吸困难组血清NT-proBNP浓度为(92.65±56.81)pg/m L;对照组血清NT-proBNP浓度为(82.9334.47)pg/m L。三组之间差异具有统计学意义(F=45.97,P<0.05)。采用Q检验两两之间多重比较,显示心源性和肺源性呼吸困难组、心源性呼吸困难组和对照组的NT-proBNP含量差异有统计学意义,而肺源性和对照组差异不显著。心源性呼吸困难组NT-proBNP阳性率为82.35%,肺源性呼吸困难组NT-proBNP阳性率为12.82%,对照组NT-proBNP阳性率为12.90%。三组阳性率具有统计学意义(χ2=77.99,P<0.05)。结论血清NT-proBNP在心源性呼吸困难和肺源性呼吸困难诊断中有重要的意义。
Objective To investigate the diagnostic significance of the N-terminal pro brain natriuretic peptide(NT-proBNP) in the diagnosis of cardiac and pulmonary dyspnea.Methods 163 patients with dyspnea were divided into 85 patients with dyspnea group and 78 cases with pulmonary dyspnea group,and 31 cases with healthy physical examination as control group.The concentrations of NT-proBNP in the three groups were detected by using the chemiluminescence method and analyzed statistically.Results Cardiac dyspnea group,the serum NT proBNP levels(218.24 + 126.99) pg/m L; pulmonary dyspnea group,the serum NT proBNP levels for(92.65 + 5.681) pg/m L; the control group,the serum levels of NT proBNP levels for(82.93 34.47) pg/m L.The difference between the three groups was statistically significant(F = 45.97,P〈0.05).Between Q test for pairwise multiple comparison,with the cardiac and pulmonary dyspnea group,cardiac dyspnea group and control group of NT proBNP levels differences have statistical significance,and pulmonary and control group,the difference was not significant.The positive rate of cardiac dyspnea group was 82.35%,and the positive rate of pulmonary dyspnea group was 12.82%,and the positive rate of control group was 12.90%.The 2 test was three,the positive rate of the group was statistically significant(χ2= 77.99,P〈0.05).Conclusion Serum NT-proBNP has important diagnostic significance in the diagnosis of cardiac dyspnea and pulmonary dyspnea.
