The value of diagnosing autoimmune pancreatitis with ultrasound

目的:通过回顾性分析自身免疫性胰腺炎(AIP)患者的临床、超声表现及病理结果,探讨AIP的超声影像特点及与相关疾病的鉴别诊断要点。方法:回顾性分析了10份病例,其中超声诊断正确3例,误诊7例,通过对误诊病例的剖析,总结AIP的临床表现和超声影像特点。结果:弥漫型AIP以胰腺弥漫性肿大和肝内胆道扩张为主要表现,局限型AIP需要抓住主胰管狭窄这一特点与胰腺癌或慢性胰腺炎鉴别,必要时还需辅以诊断性治疗帮助鉴别。结论:与其他影像方法相比,超声无创、简便、图像清晰、可重复性好,是早期发现和诊断AIP的重要手段,也是随访观察的常用方法,实验室检查亦是重要的参考指标。

Objective：Summarizing the characteristics of ultrasonic images of autoimmune pancreatitis and the essentials of differential diagnosis of relevant diseases, to improve sonographers＇ understanding of the disease and reduce the rate of misdiagnosis.Method： Review of 10 cases , of which 7 cases was misdiagnosed, and analysis of the clinical manifestations and ultrasonic images of these AIP cases.Results： Diffuse AIP features enlargement of the pancreas.Localized AIP differentiates from pancreatic cancer and chronic pancreatitis in stricture of main pancreatic duct, sometimes diagnostic treatment are necessary.Conclusion： Compared with other imaging methods, ultrasound is noninvasive, simple, clear and has a good repeatability.It is an important method for early discovery and diagnosis of AIP, common in follow-up observations and provides important referential indexes for laboratory tests.
