上颌窦底提升不植骨同期植入种植体疗效观察

Effect of maxillary sinus floor no bone grafting period implented implant

目的:探讨上颌窦底提升不植骨同期植入种植体的临床疗效。方法:选取36例上颌后牙缺失患者按收治时间顺序分为对照组18例(可用骨高度5~8mm,共种植21颗)和观察组(可用骨高度4~5mm,共种植23颗),均进行上颌窦底提升术,观察组进行上颌窦底提升不植骨同期植入种植体进行修复,对照组上颌窦提升植骨同期植入种植体进行修复,修复6个月后,比较两组种植体的存留率,随访1年,记录两组新骨获得量及牙周探诊深度。结果:两组种植体存留率比较差异无统计学意义;观察组新骨获得量(2.92±0.49)mm、牙周探诊深度(2.87±0.35)mm均与对照组相比差异不显著(P>0.05),观察组术后3周骨密度为(0.041±0.010)、4周(0.091±0.024)、8周(0.135±0.043),8周后与前期相比,差异具有统计学意义(P<0.05)。结论:上颌窦提升植骨同期植入种植体和不植骨粉同期植入种植体取得同样的治疗效果,疗效肯定,均适用于上颌后牙缺失者,具有较高成功率。

Objective： To investigate the clinical effect of implantation of the same period in the treatment of maxillary sinus floor.Methods ： 36 cases with maxillary posterior teeth missing patients according to the time sequence of were divided into control group （n=18） （5-8 mm, the planting of 21 stars） and observation group （4 - 5mm, planted a total of 23 teeth）, underwent maxillary sinus floor elevation, observation group floor of maxillary sinus lifting plant would not simultaneous bone implant repair, control group of maxillary sinus augmentation simultaneous bone implant repair, 6 months after the repair, compared two groups of implant retention rate, follow-up of 1 year recorded two new bone gain weight and periodontal probe probing depth.Results： Two groups of implant retention rate difference was not statistically significant;observation group new bone was （2.92±0.49） mm, periodontal probe probing depth （2.87±0.35） mm were with those in the control group compared with no significant difference and observation group after 3 weeks, the bone density for （0.041±0.010）, 4 weeks （0.091±0.024） and 8 weeks （0.135±0.043）, after 8 weeks, compared with the previous, the difference was statistically significant （P〈 0.01）.Conclusion： Maxillary sinus elevation with simultaneous bone implant and graft bone implants are inserted and achieve the same therapeutic effect, curative effect affirmation shall apply to the in the maxillary posterior teeth loss, has a higher success rate.
