两种低分子肝素应用方法在挽救断指再植术后静脉危象中的应用效果分析

Analysis on application effect of two different application methods of low molecular heparin in rescuing vein crisis after replantation of severed finger

目的:观察两种低分子肝素应用方法在断指再植术后静脉危象挽救中的应用效果。方法:选取2015年1月~2016年6月医院诊治的断指再植术后静脉危象的皮瓣移植患者60例,随机等分为对照组和观察组。对照组在脐周皮下注射低分子肝素并对淤血区小切口放血,观察组在淤血部位皮下注射低分子肝素并做小切口淤血放血,2组患者均给予系统化护理干预,比较2组患者防治效果。结果:观察组患者术后2 h凝血酶时间显著低于对照组;观察患者术后1 d,5 d,7 d肿胀直径显著低于对照组;观察组术后并发症发生及皮瓣坏死情况显著低于对照组,差异均有统计学意义(P<0.05)。结论:断指再植术后静脉危象患者在淤血部位皮下注射低分子肝素并进行小切口淤血放血治疗,能降低静脉危象发生率,值得推广应用。

Objective：To observe the application effect of two application methods of low molecular heparin in rescuing vein crisis after replantation of se- vered finger. Methods：Selected 60 skin flap transplantation patients with vein crisis after replantation of severed finger admitted in our hospital from January 2015 to June 2016, and they were equally divided into control group and observation group according to the rescue method. The patients in the control group have periderm subcutaneous injection of heparin, and bleeding of small incision at the blood congested area, and patients in the observation group had sub- cutaneous injection of heparin in the blood congested area, and received bleeding at the congested area of small incision, patients in the two groups were both given systematical nursing intervention, and the prevention and treatment effect of patients in both groups was compared. Results ： The thrombin time of patients in the observation group 2h after operation was significantly lower than that of patients in the control group ; the swelling diameters of patients in the observation group 1 d, 5d and 7d after operation were significantly lower than that of patients in the control group; and the post -operation complications of patients in the observation group were significantly lower than those of patients in the control group, and the difference was statistically significant （ P 〈 0.05 ）. Conclusion：The subcutaneous injection of heparin in the blood congested area and small incision bleeding for the patients with vein crisis after re- plantation of severed finger could reduce the incidence of vein crisis and was worth promoting clinically.
