快速康复理念在全腹腔镜下结直肠癌微创手术中的应用

Application of fast track surgery in laparoscopic minimally invasive surgery of colorectal carcinoma

目的观察快速康复理念在全腹腔镜下结直肠癌微创手术中的有效性和安全性。方法选取2014年1月—2016年12月行全腹腔镜下根治性手术的78例结直肠癌为观察组,同期择期行全腹腔镜下根治性手术的75例结直肠癌为对照组,对照组予传统围手术期处理措施,观察组采用快速康复外科流程。比较两组手术时间,术中出血量,肛门排气、术后进食、术后下地、住院时间等。结果观察组肛门排气时间、术后进食时间、术后下地时间及住院时间均短于对照组,术后并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论快速康复理念应用于全腹腔镜下结直肠癌微创手术可促进患者康复,减少术后并发症。

Objective To observe the efficacy and security of fast track surgery in laparoscopic minimally invasive surgery of colorectal carcinoma. Methods A total of 78 cases who underwent laparoscopic minimally invasive surgery of colorectal carcinoma from January 2014 to December 2016 were selected as observation group, and another 75 cases who underwent the same surgery of colorectal carcinoma in the same period were selected as control group. The controls were given conventional perioperative treatment, and the observation group received fast track surgery. Operation time, intraoperative blood loss, anal aerofluxus time, postoperative feeding time, ambulation time, and duration of hospital stays were compared between the two groups. Results In the observation group, the anal aerofluxus time, postoperative feeding time, ambulation time, and duration of hospital stays were shorter than those of the control group; and the complication rate was also lower （P〈0.05）. Conclusion Fast track surgery in laparoscopic minimally invasive surgery can promote recovery and reduce postoperative complications of patients with colorectal carcinoma.
