血清Irisin水平与妊娠期糖尿病的相关性研究

Relationship between serum Irisin level and gestational diabetes mellitus

目的探讨血清Irisin水平与GDM患者IR及糖脂代谢的关系。方法选取在湖北医药学院附属东风医院产科住院的足月妊娠(≥37周)单胎孕妇80名,其中GDM患者40例(GDM组),N(T孕妇40名(GNGT组)。ELISA测量两组血清Irisin水平;同时测定FPG、FIns、HbA_1c、TG、TC、HDL-C、LDL-C水平。结果 GDM组血清Irisin水平较GNGT组[(7.35±1.85)vs(6.11±1.54)ng/ml]降低(P<0.05),且与HOMA-IR、FIns呈负相关(r=-0.370、-0.474,P=0.019)。多元线性回归分析显示,GDM组FIns和TC是血清Irisin的独立影响因素。结论血清Irisin可能参与了GDM的发生,与GDM患者IR及糖脂代谢异常有关。

Objective To investigate the relationship between serum Irisin level and insulin resistance （IR）and glucose and lipid metabolism in patients with gestational diabetes mellitus （GDM） . Methods From January 2015 to June 2016, a total of 80 pregnant women with full-term single pregnancy were erooled in Hubei university of medicine affiliated Dongfeng obstetric hospital, including 40 cases with GDM （GDM group） and 40 with gestational normal glucose tolerance （GNGT group）. The serum Irisin levels were measured by ELISA method. Fasting blood glucose, fasting insulin, glycosylated hemoglobin, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol and triglyceride were measured at the same time. Results Compared with GNGT group[（7. 35±1.85） ng/ml], the serum levels of Irisin in GDM group [ （ 6.11 ± 1.54） ng/ml-] were significantly lower （P = 0. 002）. Serum Irisin negatively correlated with HOMA-IR （r=-0. aT0,P=0. 019） and fasting insulin （r=-0. 474,P= 0. 002）. Multiple stepwise regression showed that fasting insulin and triglyceride in GDM group were independent predictors of serum Irisin. Conclusion Serum Irisin may be involved in the occurrence of GDM by affecting insulin resistance and glucose-lipid metabolism.
