摘要
目的探讨血清胃蛋白酶原Ⅰ(PGⅠ)、PGⅠ/PGⅡ(PGR)和血清胃癌相关抗原(MG7-Ag)在胃癌诊断中的意义。方法根据胃镜和组织病理学检查结果将120例胃镜受检者分为3组。正常对照组:胃黏膜正常或非萎缩性胃炎50例;胃癌前病变组:萎缩性胃炎及肠化生、异型增生组30例;胃癌组40例。分别对3组应用增强免疫比浊法定量测定PGⅠ和PGⅡ水平,同时应用ELISA方法定量测定MG7-Ag水平。结果胃癌组PGⅠ、PGR水平低于胃癌前病变组及正常对照组(P<0.05);胃癌组MG7-Ag水平高于胃癌前病变组及正常对照组(P<0.05)。胃癌患者血清PGⅠ+PGR、MG7-Ag二者联合检测(并联法)阳性率为89.87%,胃癌前病变组的检出率为57.77%,高于单独检测PGⅠ+PGR或MG7-Ag的阳性率(P<0.05)。结论联合检测血清中PG、MG7-Ag水平对胃癌诊断具有重要意义。
Objective To explore the clinical value of determination of serum pepsinogen (PG Ⅰ ), PG Ⅰ/PG Ⅱ (PGR) and MGT-Ag for diagnosis of gastric cancer. Methods A total of 120 patients were divided into 3 groups based on endoscopic and histopathological findings: 30 patients in atrophic gastritis group, 40 in gastric cancer group, and 50 in control group (with normal gastric mucosa or non-atrophic gastritis). MGT-Ag was detected by enzyme linked immunosorbent assay (ELISA), and serum PG Ⅰ and PG Ⅱ levels were measured by latex enhanced immune turbidimetry. Results PG Ⅰ and PGR levels of the gastric cancer group were significantly lower than those in the atrophic gastritis group and the control group (P 〈 0. 05). MG7-Ag level of the gastric cancer group was remarkably higher than that in the atrophic gastritis group and the control group (P 〈 0. 05). The positive expression rate of combined detection of serum MG7-Ag and PG level was 89.87% in the gastric cancer group and 57.77% in the atrophic gastritis group, which was significantly higher than the positive rate of the detection of PG Ⅰ +PGR or MG7-Ag in the corresponding group (P 〈 0.05). Conclusions Combined detection of serum MG7-Ag and PG has an important value in the diagnosis of gastric cancer.
