氟哌啶醇联合氯氮平治疗难治性精神分裂症的疗效及安全性临床评价

Efficacy and Safety of Haloperidol combined with Clozapine in the Treatment of Refractory Schizophrenia

目的探讨氟哌啶醇联合氯氮平治疗难治性精神分裂症的临床疗效和安全性。方法选择医院2013年2月至2016年2月收治的60例难治性精神分裂症患者,随机分为对照组和观察组,各30例。对照组患者给予氯氮平,观察组患者在此基础上给予氟哌啶醇进行治疗,比较两组患者的阴性和阳性综合征量表(PANSS)评分,血常规检测结果,临床疗效,锥体外系反应评分及不良反应发生情况。结果两组患者治疗前、治疗后1周和2周PANSS评分比较,差异不显著(P>0.05),治疗后4,8,12周比较,差异显著(P<0.05);两组治疗后红细胞(RBC)、白细胞(WBC)下降,淋巴细胞(LY)升高,组间比较,差异不显著(P>0.05);治疗后,对照组和观察组帕金森综合量表(SAS)评分比较,差异显著(P<0.05);观察组临床疗效和不良反应发生率为93.33%和6.67%,优于对照组的73.33%和30.00%(P<0.05)。结论氯氮平联合氟哌啶醇治疗难治性精神分裂症的疗效显著,安全性高,值得临床推广。

Objective To analyze the clinical efficacy and safety of haloperidol combined with clozapine in the treatment of refractory schizophrenia. Methods 60 patients with refractory schizophrenia admitted in the hospital from February 2013 to February 2016 were divided into two groups, 30 cases in each group. The control group received clozapine; on this basis, the observation group was added with haloperidol. The PANSS score, blood routine test results, clinical efficacy, extrapyramidal side effects score and adverse reactions in the two groups were compared. Results There was no significant difference in PANSS score between the two groups before treatment and 1, 2 weeks after treatment （ P 〉 0. 05 ）;there were significant differences between the two groups in 4, 8 and 12 weeks after treat-ment （ P 〈 0. 05 ）;after treatment, RBC, WBC decreased, LY increased, with no significant difference between the two groups （ P 〉 0. 05 ）;the SAS scores of the control group and the observation group were statistically different （ P 〈 0. 05 ）;the clinical efficacy and adverse reaction rate of the observation group were 93. 33% and 6. 67% , which were better than those of the control group （ 73. 33% vs 30. 00%, P 〈 0. 05 ） . Conclusion The combination of clozapine and haloperidol is effective and safe in the treatment of refractory schizophrenia, and is worthy of clinical promotion.
