Characteristics and the direct economic loss of nosocomial infections in esophageal cancer surgical patients

摘要 目的分析某综合医院食管癌手术患者医院感染特征与直接经济损失,为做好医院感染预防与控制工作提供依据。方法回顾性调查医院2014年7月-2016年6月出院的156例食管癌手术患者相关资料,分析患者医院感染情况及其所致的直接经济损失。结果医院感染率为32.69%、例次感染率为37.18%;共检出医院感染病原菌44株,其中革兰阴性菌占75.00%;医院感染患者住院总费用(中位数)为69 343.92元,显著高于未感染患者54504.17元(P<0.05);下呼吸道感染、手术部位感染(SSI)、下呼吸道感染并SSI患者的住院天数和住院费用有统计学差异(P<0.05)。结论食管癌手术患者医院感染率较高,造成的经济损失巨大,不同医院感染类型所致的经济损失有所不同,做好医院感染预防与控制工作意义重大。 OBJECTIVE To investigate the characteristics and the direct economic losses of esophageal cancer surgical patients due to nosocomial infections in a general hospital,so as to provide evidence for prevention and control of nosocomial infections.METHODS A retrospective investigation was conducted to collect information of 156 esophageal cancer surgical patients discharged from Jul.2014 to Jun.2016.The characteristics of nosocomial infection and the direct economic losses caused by nosocomial infections were analyzed.RESULTS The infection rate and case-time infection rate were 32.69% and 37.18%,respectively.In total,44 strains of pathogens of nosocomial infections were detected,and gram-negative bacteria accounted for 75.00%.The cost(median)of patients with nosocomial infections(67456.25yuan)was significantly higher than patients without infection(54504.17yuan)(P〈0.05).The hospitalization duration and the cost of lower respiratory tract infection,surgical site infection(SSI),lower respiratory tract infection combined with SSI were significantly different(P〈0.005).CONCLUSIONThe nosocomial infection rate of esophageal cancer surgical patients is high,and can cause huge economic losses.The economic losses caused by different types of nosocomial infections are different.It is of great significance to prevent and control the nosocomial infections.
