双极射频消融治疗室壁瘤相关性室性心律失常对心率变异性的影响 被引量:8

摘要 目的探讨双极射频消融治疗室壁瘤相关性室性心律失常对心率变异性的影响。方法选取2009年9月至2014年12月首都医科大学附属北京安贞医院心外六病房共14例接受不停跳冠状动脉旁路移植术和室壁瘤成形术联合双极射频消融的患者,比较手术前后患者心率变异性。结果患者手术前与手术后相邻正常心率周期相差值〉50ms的个数所占的百分比、相邻正常心率周期相差值均方的平方根、24h内全部正常心率周期计算的标准差、每5分钟平均RR间期计算的标准差比较[2.5%(1.0%,6.0%)比3.O%(2.0%,5.0%)、63.0(40.0,93.0)ms比58.5(29.0,87.0)ms、43.0(35.0,63.0)ms比48.0(35.0,65.0)ms、98.5(91.0,125.0)ms比100.0(92.0,126.0)ms](均P〉0.05)。结论双极射频消融治疗室壁瘤相关性室性心律失常的方法在术后早期对心率变异性无明显影响,其机制及远期影响尚待进一步研究。 Objective To discuss the effect of bipolar radiofrequency ablation treating ventricular aneurysm related ventricular arrhythmia on heart rate variability. Methods Fourteen patients who had off-pump coronary artery bypass grafting and left ventricular aneurysm repair combined with bipolar radiofrequency ablation from September 2009 to December 2014 in Beijing Anzhen Hospital, Capital Medical University were enrolled. Preoperative and postoperative heart rate variabilities were analyzed. Results There were no significant differences of the percentage of intervals 〉 50 ms different from preceding interval, square root of the mean of the squares of successive NN interval differences, standard deviation of normal RR intervals and standard deviation average 5 min RR intervals before and after operation[ 2.5% ( 1.0%, 6.0% ) vs 3.0% (2.0% , 5.0% ), 63.0 (40.0, 93.0)ms vs 58.5(29.0, 87.0)ms, 43.0(35.0, 63.0)ms vs 48.0(35.0, 65.0)ms, 98.5(91.0, 125.0 ) ms vs 100.0 (92.0, 126.0) ms ] ( all P 〉 0. 05 ). Conclusion Bipolar radiofrequeney ablation for ventricular aneurysm related ventricular arrhythmia has no significant postoperative early effect on heart rate variability.
