贞芪扶正颗粒治疗大肠癌术后癌因性疲乏临床研究

Clinical Study on the Treatment of Cancer Related Fatigue in Patients of Postoperative Large Intestine Cancer with Zhenqi Fuzheng Granules

目的:观察贞芪扶正颗粒治疗大肠癌术后癌因性疲乏(cancer-related fatigue,CRF)的临床疗效及对免疫功能的影响。方法:120例患者随机分为对照组和试验组,每组各60例。对照组给予对症处理,试验组在对照组基础上加用贞芪扶正颗粒,每次10 g,开水冲服,每天3次,连续治疗2个月。比较两组患者治疗前后Piper疲乏量表评分、生活质量改善情况和疲乏程度,检测T淋巴细胞亚群和自然杀伤细胞水平,评价临床疗效及化疗不良反应。结果:试验组中医证候疗效有效率为91.67%,高于对照组的76.67%,差异有统计学意义(P<0.05);试验组生活质量改善情况优于对照组,差异有统计学意义(P<0.05);治疗后试验组Piper疲乏量表评分各维度和总分均低于对照组(P<0.01),且疲乏程度轻于对照组(P<0.05);治疗后试验组自然杀伤细胞、CD_3^+、CD_4^+及CD_4^+/CD_8^+比值高于对照组,CD8+低于对照组,差异有统计学意义(P<0.01);试验组化疗导致的不良反应程度轻于对照组(P<0.01)。结论:贞芪扶正颗粒治疗大肠癌术后CRF患者,能显著降低Piper疲乏量表评分,改善CRF症状和中医证候,提高患者的生活质量和机体的免疫功能,减轻化疗不良反应程度。

Objective：To observe the clinical effect of Zhenqi Fuzheng Granules treatment of cancer related fatigue in patients of postoperative large intestine cancer and its effect on the immune function.Methods：120 patients were randomly divided into the control group and the experimental group,each group of 60 cases.The control group was given symptomatic treatment,and the experimental group was treated with Zhenqi Fuzheng Granules based on the control group,10 g each time,3 times a day for 2 months.Compared Piper Fatigue Scale score,quality of life improvement and fatigue level of the two groups before and after treatment,detected T lymphocyte subsets and natural killer cells,and evaluated the clinical efficacy and adverse reactions.Results：the effective rate of TCM syndrome in the experimental group was 91.67%,which was higher than that of the control group 76.67%,and the difference was statistically significant（P 0.05）.The quality of life in the experimental group was better than that in the control group（P 0.05）.After treatment,the scores of the Piper fatigue scale of the experimental group were lower than those of the control group（P 0.01）,and the degree of fatigue was less than that of the control group（P 0.05）.After treatment,natural killer cells,CD_3~＋,CD_4~＋ and CD_4~＋/CD_8~＋ratio in the experimental group was higher than that in the control group,CD_8~＋ was lower than that in the control group,the difference was statistically significant（P 0.01）;the adverse reaction of chemotherapy in theexperimental group was lighter than that in the control group（P 0.01）.Conclusion：Zhenqi Fuzheng Granules in the treatment of CRF patients with colorectal cancer after operation can significantly reduce the Piper Fatigue Scale score,improve CRF symptoms and TCM syndrome,improve the immune function of the patient＇ s quality of life and body,and reduce the adverse reaction of chemotherapy.
