温经通督药熥法配合艾灸治疗寒湿闭阻型腰痛临床研究

Clinical Research on Warming Meridian and Activating Du Chinese Medicine Tong Method Combined with Moxibustion Point Weizhong Treatment of Cold-Damp Block Type Lumbago

目的:观察中药包外熥配合艾灸委中穴治疗寒湿闭阻型腰痛的临床疗效,并探讨其效应与作用机制。方法:选取96例确诊为寒湿闭阻型腰痛患者随机分为治疗组和对照组各48例,治疗组采用中药包循经外熥配合艾灸委中穴治疗,对照组给予口服洛索洛芬钠片及腰椎推拿治疗,两组均以10 d为1个疗程,分别于第1个疗程、第2个疗程后观察两组患者腰痛相关症状、体征等改善情况,通过观察两组患者视觉模拟评分法(visual analogue scale,VAS)评分、日本腰椎骨科协会下腰痛(japanese orthopaedic association scores,JOA)评分和Oswestry功能障碍指数问卷表(oswestry disability index,ODI)评分的变化情况对其效应进行分析与评估,于1个月后采用患者自填健康调查表(MOS 36-ltem short form health survey,SF-36)评估生存质量,并分析各组临床疗效,3个月后随访分析各组复发率。结果:两组患者VAS评分和ODI评分均有所减少,较对照组,治疗组减少更为明显(P<0.05);两组患者JOA评分中主观症状、临床体征和日常活动受限度比较,其评分均有增加,较对照组,治疗组JOA评分增加更为明显(P<0.05),两组膀胱功能比较,差异无统计学意义(P>0.05);两组患者SF-36评分均增高,较对照组,治疗组增高更为明显(P<0.05);治疗组效率为93.75%,治愈率68.75%,3个月后复发率4.17%,均显著优于对照组(P<0.05)。结论:温经通督药熥法配合艾灸委中穴治疗寒湿闭阻型腰痛,疗效显著。

Objective：To observe the clinical effect of Chinese medicine Tong agent for external use combined with moxibustion point Weizhong treatment of cold-damp block type lumbago,and to investigate the effect and mechanism of action.Methods：96 cases diagnosed with cold-damp block type lumbago patients were randomly divided into the treatment group and the control group,each of 48 cases.The treatment group was treated with Chinese medicine Tong agent for external use combined with moxibustion point Weizhong,and the control group was given Loxoprofen Sodium Tablets in acute stage,and lumbar massage therapy in stable phase.The two groups were treated with 10 days for a total of 1 course,respectively,after the first and the second course of the treatment,the symptoms and signs were observed in the two groups.Analyzed and evaluated by observing two groups of patients with visual analogue scale（VAS） score,Japanese Orthopaedic Association scores（JOA）,Oswestry disability index（ODI） ques-tionnaire.After 1 month,MOS 36-ltem short form health survey（SF-36） was used to assess the quality of life,clinical efficacy and the recurrence rate of each group were analyzed.Results：The two groups of patients with VAS score and ODI score were reduced,especially in the treatment group was significantly reduced（P 0.05）.The two groups of patients with JOA score in subjective symptoms,clinical signs and daily activities by the limit,the scores were increased,the treatment group JOA score increased the most obvious（P 0.05）,compared with the two groups of bladder function,there was no statistical significance（P 0.05）.The SF-36 scores of the two groups were all increased,especially in the treatment group（P 0.05）.The total effective rate of the treatment group was 93.75%,the cure rate was 68.75%,and 3 months after the recurrence rate of 4.17%,which was better than that of the control group（P 0.05）.Conclusion：Warming meridian and activating Du Chinese medicine Tong method combined with moxibustion point Weizhong treatment of cold-damp block type lumbago has significant effect.
