摘要
目的:评价循证医学教学模式在神经内科住院医师规范化培训中的应用效果。方法:2012年9月—2015年9月参加神经内科住院医师规范化培训的学员共62人,随机分为两组。常规教学模式组31人,采用常规教学法。EBM教学模式组31人,在常规教学法的基础上融入EBM理念。24周临床轮转结束后进行理论考试和临床技能考核,并调查学员对两种教学模式的满意程度。结果:两组学员的理论考试成绩无明显差异(79.06±7.28分对80.94±5.21分,P=0.2491),但EBM教学模式组学员临床考核成绩显著优于常规教学模式组(84.52±8.19分对76.68±9.18分,P<0.05)。此外,相较于常规教学模式组,EBM教学模式组学员对当前教学模式的满意度更高(87.1%对64.52%,P<0.05)。结论:EBM教学模式更易被学员接受,有助于培养学员临床思维并提高其解决临床问题的综合能力,值得在神经内科及其他专科住院医师规范化培训的临床教学中推广应用。
Objective: to evaluate the effect of evidence-Rased medicine ( EBM) training approach on thestandardized training for neurology residents. Methods: a total of 62 neurology residents ( during 09/01/2012-09/ 01/2015) was selected,and was randomly divided into two groups: classic group (31 persons, receiving classictraining) and EBM group (31 persons, receiving EBM training). Twenty-four weeks later, basic theory and clinical skills examinations were used to evaluate the difference between classic training approach and EBM training approach. In addition,a survey was conducted to assess the satisfaction level of residents with these two differenttraining approaches. Results: no diference was observed in scores of basic theory examinations between classic group and EBM group (79. 06 ±7. 28 points vs. 80. 94 ±5.21 points, P=0. 2491 ). Residents in EBM group had a significantly higher score in the clinical skils examination when compared witli that in classic group ( 84. 52 ± 8. 19 poins vs. 76. 68 ± 9. 18 points, P〈 0. 05 ). In addition, mean satisfaction level witii training approach in EBM group was significantly higher than that in classic group (87. 1% vs. 64. 52% , P〈0. 05 ) . EBM training approach is well accepted by neurology residents and can substantially improve their comprehensiveclinical abilities. This approach deserves further generalization and application in standardized training for residents in China.
