CT灌注成像在老年人脑梗死血流动力学研究中的应用价值

The Value of CT Perfusion Imaging on Cerebral Hemodynamics of the Aged with Cerebral Infarction

目的探讨在老年性脑梗死脑血流动力学研究中CT灌注成像(CTPI)的应用价值。方法拟诊为脑梗死的患者50例,在发病1天内给予CTPI检查和16层CT平扫,对兴趣区脑血流量(CBF)、对比剂峰值时间(TTP)以及对比剂平均通过时间(MTT)进行测定,同时和对侧相应脑组织灌注参数进行对比;3~10天后全部患者均予MRI随访。结果全部50例患者,20例患者经CT平扫检查发现缺血灶,47例患者经CTPI检查发现异常灌注。和对照区域相比,缺血区CBF显著降低、TTP、MTT显著延长(P<0.05)。结论 CTPI能准确反映脑梗死血流动力学状态,可为早期诊断和治疗老年性脑梗死提供科学依据。

Objective To investigate the value of CT perfusion imaging （ CTPI） in the hemodynamic study on the senile cerebral in-farction. Methods 50 patients doubted with cerebral infarction were tested by 16-slice CT plain scan and CTPI within 24 hours of on- set.The cerebral blood flow（ CBF ）, time to peak （ TTP ） and mean transit time （ MTT ） in region of interest （ ROI ） were determined and compared with the perfusion parameters in the contralateral corresponding area. All cases were followed-up with MRI after 3-10 days. Results Out of all cases, ischemia lesion was found on CT plain scanning in 20 patients and 47 patients showed abnormal perfu-sion on CTPI.In contrast to the control area, CBF was significantly reduced while MTT and TTP were significantly increased in re-search area （ P〈 0.05 ）. Conclusions CT perfusion imaging can accurately reveal the hemodynamic condition of cerebral infarction, which could provide a scientific basis for early diagnosis and treatment of the senile brain infarction .
