血液细胞形态学检查在血常规检测应用中的临床价值

Clinical Value of Blood Morphocytology Examination in the Blood Routine Test

目的探讨血液细胞形态学检查在血常规检测中的应用价值。方法筛选2014年9—12月该院检验科全自动血液分析仪监测结果异常的血液标本530例作为观察组,另选取431例检测结果正常的血液标本作为对照组,对比两组血液标本中血液细胞形态学检查结果,研究血液细胞形态学检查在血常规检测中的应用价值。结果 431例对照组血液标本中血液细胞形态异常的标本共计19例,计算假阴性率为4.41%,采用全自动血液分析仪联合血液细胞形态学检查共检出形态异常146例,检出阳性率为33.87%;530例观察组标本中血液细胞形态正常共计175例,假阳性率为33.02%;在对对照组异常血液标本结果分析中发现,19例形态异常的标本主要以白细胞形态异常为主,与其他异常指标比较差异无统计学意义(P<0.05)。结论虽然传统血细胞涂片细胞形态学检查具有操作繁琐、检测效率低下的缺点,但在血常规检查中联合应用全自动血细胞分析仪可有效提升患者临床诊断正确率,避免漏诊现象的发生,可为临床对疾病诊断以及治疗提供更有价值的参考信息。

Objective To discuss the clinical value of blood morphocytology examination in the blood routine test. Methods530 cases of blood specimens with abnormal Results by the full-automatic blood analyzer in the department of clinical laboratory in our hospital from September to December 2014 were selected as the observation group, and 431 cases of blood specimens with normal blood specimens were selected as the control group, and the morphocytology examination results were compered between the two groups, and the application value of it in the blood routine test was researched. Results In the control group, there were 19 cases with abnormal specimens, and the false-negative rate was 4.41%, Using automatic blood analyzer combined detection of blood cell morphological examination, a total of morphological abnormalities in 146 cases, check out the positive rate is 33.87%; Specimens of 530 cases of observation group a total of 175 cases of normal blood cells in the form, and the false positive rate was 33.02%, and in the control group, white cell paramorphia was the main in the 19 cases with abnormal specimens, and the difference had no statistical significance（P〈0.05）. Conclusion Even though the traditional blood cell smear morphocytology examination has the shortcomings of complex operation and low efficacy of test, the combined application of it with automated hematology analyzer can effectively improve the clinical diagnosis accurate rate and avoid the occurrence of misdiagnosis, which can provide more valuable information for the disease diagnosis and treatment in clinic.
