摘要
目的系统评价社区干预对2型糖尿病疗效的影响。方法检索万方数据库、维普资源信息系统、中国期刊全文数据库、中国生物医学文献数据库及Pubmed数据库中2006年以来发表的有关社区干预对我国2型糖尿病疗效影响的相关文献。分析软件采用Stata 12.0。结果共纳入22篇文献,累计干预组患者1 723例,对照组患者1 674例。Meta分析结果显示,干预组空腹血糖(SMD=0.726,95%CI:0.359~1.092),餐后2 h血糖(SMD=0.798,95%CI:0.407~1.127)、糖化血红蛋白(SMD=0.735,95%CI:0.251~1.219)、总胆固醇(SMD=0.476,95%CI:0.098~0.855)、甘油三酯(SMD=0.721,95%CI:0.227~1.214)均低于对照组,差异均有统计学意义(均P<0.05)。结论社区干预对2型糖尿病病情控制效果显著,建议对2型糖尿病患者采取积极的社区干预措施。
Objective To evaluate the effectiveness of community-based interventions among type 2 diabetes mellitus. Methods Literatures regarding the effectiveness of combined community-based interventions among type 2 diabetes in China since 2006 were collected through databases including Wanfang Med Online, VIP Chinese Science and Technology Periodical Databases, China National Knowledge Infrastructure (CNKI), Chinese Biomedical Literature Databases( CBM), and PubMed Databases. Data were analyzed with Stata 12.0 software . Results A total of 22 articles were included in the study, containing 1 723 eases in the intervention group and 1 674 cases in the control group. The fasting blood-glucose (FBG) ( SMD = 0. 726,95 % CI: 0. 359 - 1. 092) , meal after 2 h blood glucose(P2hBG) ( SMD = 0. 798, 95% CI: 0. 407 - 1. 127 ), glycated hemoglobin (HbAlc) ( SMD = 0. 735,95% CI: 0. 251 - 1. 219 ), total cholesterol (TC) ( SMD = 0. 476,95 % CI: 0. 098 - 0. 855 ), triglyceride ( TG ) ( SMD = 0. 721,95 % CI: 0. 227 - 1. 214)in the community-based intervention group were lower than those in the control group (P 〈 0.05 for all). Conclusion The community-based interventions had significant effectiveness on control of type 2 diabetes. Active community-based interventions should be recommended for type 2 diabetes.
