高频超声在外周神经损伤中的应用价值

Application value of high frequency ultrasonography in peripheral nerve injury

目的探讨高频超声在外周神经损伤中的应用价值。方法对47例外周神经损伤患者采用高频超声进行检查,归纳这类患者的超声声像图特征。结果本组47例患者经手术确诊为完全断裂16例,部分断裂18例,卡压13例,超声检查结果与手术确诊结果进行对照分析,发现超声诊断符合率为95.74%(45/47),误诊2例,误诊率为4.26%(2/47)。其中1例外周神经卡压误诊为外周神经完全断裂,另外1例患者经手术探查疝气补片卡压,而超声见股神经正常。结论高频超声检查可以准确判断外周神经损伤的部位和类型,为诊断治疗提供有效的帮助。

Objective To explore the application value of high frequency ultrasonography in peripheral nerve injury. Methods 47 patients with peripheral nerve injury were examined by high frequency ultrasonography, and the sonographic features were compared with intraoperative exploration results. Results Among 47 patients confirmed by surgery, there were 16 eases with complete fracture, 18 cases with partial fracture and 13 cases with compression, the diagnostic accuracy rate was 95.74% （45/47）, 2 cases were misdiagnosed, the misdiagnosis rate was 4.26% （2/47）. 1 case of peripheral nerve entrapment was misdiagnosed as complete fracture, 1 case with hernia patch clamping compression was missed diagnosis. Conclusion High frequency uhrasonography can accurately determine the location and the type of peripheral nerve injury, thus being helpful in clinic preoperative evaluation and surgical interevention, and it can be used as a preferred inspection method of peripheral nerve injury.
