摘要
儿童及青少年甲状腺癌是一种少见的恶性肿瘤,以分化型甲状腺癌为主,临床病理特征及预后与成人甲状腺癌有所不同,其侵袭性较强、易出现局部淋巴结转移及远处转移。儿童及青少年甲状腺癌多表现为颈部无痛性肿块,结合超声及细针穿刺细胞学检查可明确诊断。虽然儿童及青少年甲状腺癌发现时多分期较晚,但总体预后较好。手术是最主要的治疗方式,但手术方式仍存在争议,此外其他治疗方法包括内分泌治疗、放射性^(131)I治疗、分子靶向治疗等。
The thyroid carcinoma in children and adolescents is rare and most of them are differentiated type. The clinical pathological features and prognosis are different from those of adult patients with thyroid carcinoma,which is highly invasive and prone to local lymph node metastasis and distant metastasis. Thyroid carcinoma in children and adolescents is manifested as painless mass in the neck, which can be diagnosed by ultrasonography and fine needle aspiration cytology and pathology. Although thyroid cancer in children and adolescents is mostly at advanced stage when discovered, the overall prognosis is good. Surgery is the main method of treatment, but the operation mode is still controversial, and other treatment methods include endocrine therapy,radiation therapy ^131I and molecular targeted therapy.
