根治性切除术对中晚期胆囊癌患者远期疗效的影响

Effect of Radical Resection on the Prospective Efficacy in Patients with Middle-late Gallbladder Cancer

目的探讨根治性切除术对中晚期胆囊癌患者远期疗效的影响。方法选取2009年1月至2012年1月陕西省核工业二一五医院收治的中晚期胆囊癌患者100例,根据随机数字法将患者分为A组、B组、C组、D组等四组,其中A组(24例)实施根治性切除术、B组(25例)实施姑息性切除术、C组(23例)实施剖腹探查术、D组(28例)实施单纯胆管外或胆管内引流术。比较不同手术方式治疗后患者中位生存以及并发症发生情况。结果A组患者的中位生存时间长于B组、C组、D组(P<0.05);术后A、B、C、D四组患者并发症发生率比较差异无统计学意义[33.33%(8/24)比36.00%(9/25)比30.43%(7/23)比28.57%(8/28)](P>0.05)。结论根治性切除术能明显延长中晚期胆囊癌患者术后的生存时间,远期疗效较好,并发症发生率低,安全性高。

Objective To study the effect of radical resection on the prospective efficacy in patients with middle-late gallbladder cancer. Methods A total of 100 cases of middle-late stage gallbladder cancer in Nuclear Industry 215 Hospital of Shaanxi from Jan. 2009 to Jan. 2012 were included, according to the random number table method, the patients were divided into group A, group B, group C, group D. Group A （24 cases） was treated by radical resection, group B （25 cases） was treated by paUiative resection, group C （23 cases） was treated by laparotomy, and group D （28 cases） was treated by simple bile duct（ outside or inside bile duct） drainage. The median survival time and complications of the four groups were compared. Results The median survival time of group A was longer than that of group B, C, D （ P 〈 0.05 ）. The overall complications incidence of group A had no significant difference from group B, C, D [33.33% （8/24） vs 36.00% （9/25） ,30.43% （7/23） ,28.57% （8/28） ] （P 〉0.05）. Conclusion Radical resection can obviously prolong the survival time of patients with mid-late stage gallbladder cancer, with good prospective efficacy, low complication rate, and high safety.
