摘要
目的比较汉防己甲素单独使用及分别联合虫草菌粉和胸腺肽治疗特发性肺间质纤维化的临床疗效。方法采用回顾性分析研究方法,选择在本院住院的180例特发性肺间质纤维化患者,随机分为三组各60例,比较三组特发性肺间质纤维化患者治疗前后的相关指标及临床疗效。计数资料比较采用χ~2检验,计量资料比较采用方差分析,两两比较采用LSD法,P<0.05为差异有统计学意义。结果胸腺肽组治疗总有效率为96.7%,虫草组为95.0%,均明显优于对照组的80.0%,三组间比较,差异有统计学意义(P<0.05)。出院时三组患者的PO_2、PCO_2、肺总量(total lung capacity,TLC)、肺活量(vital capacity,VC)及一氧化碳弥散量(carbon monoxide diffusing capacity,DLCO)指标[(69.13±6.74)、(43.26±5.11)mm Hg(1 mm Hg=0.133 kP)、(63.16±11.44)、(66.25±11.23)L、(54.16±6.85)ml·mm Hga^(-1)·min^(-1),(74.16±7.65)、(46.16±6.10)mm Hg、(73.10±10.80)、(72.06±11.20)L、(57.11±8.60)ml·mm Hg^(-1)·min^(-1),(73.16±7.55)、(45.16±6.12)mm Hg、(72.13±10.96)、(71.06±11.26)L、(58.11±8.62)ml·mm Hg^(-1)·min^(-1)]比较,差异均有统计学意义(均P<0.05)。虫草组,胸腺肽组患者出院时PO_2、PCO_2、TLC、VC及DLCO指标均显著优于对照组,差异均有统计学意义(均P<0.05)。三组患者肺CT改善情况[51(85.0%)、52(86.7%)、41(68.3%)]比较,差异有统计学意义(P<0.05)。结论采用汉防己甲素分别联合虫草菌粉和胸腺肽治疗特发性肺间质纤维化优于单用汉防己甲素,值得临床研究和推广。
Objective To compare the clinical efficacy of tetrandrine combined with cordyeeps sinensis powder and thymosin in the treatment of idiopathic pulmonary fibrosis.Methods Retrospective analysis of 180 patients with idiopathic pulmonary fibrosis in our hospital were randomly divided into three groups of 60 patients, the relevant indicators and clinical efficacy of idiopathic pulmonary fibrosis patients before and after treatment in three groups were compared. Count data was used by chi square test. Measurement data was processed by variance analysis, pairwise comparison was used by LSD. P〈0.05 was considered statistically significant.Results The total effective rate was 96.7% in the thymosin group and 95.0% in the cordyceps group, which was significantly better than the control group (80.0%)(P〈0.05). There were significant differences in PO2, PCO2, total lung capacity(TLC),vital capacity(VC)and carbon monoxide diffusing capacity (DLCO) of three groups (all P〈0.05). The levels of PO2, PCO2, TLC, VC and DLCO were significantly higher in the Cordyceps group and thymosin group than those in the control group (all P〈0.05). There was significant difference in the improvement of lung CT[51(85.0%),52(86.7%),41(68.3%)] in three groups (P〈0.05). Conclusion The combination of tetrandrine and cordyceps sinensis powder, thymosin in treatment of idiopathic pulmonary interstitial fibrosis are better than that of tetrandrine alone. It is worthy of clinical research and popularization.
