两种留置针退针芯法在老年心血管病患者中的应用效果比较

Application Effects of Two Methods of Back Indwelling Needle Core in Elderly Patients with Cardiovascular Disease

目的比较两种留置针退针芯法在老年心血管病患者中的应用效果。方法 2015年1-12月,便利抽样法选取在上海市金山区亭林医院心血管内科住院的患者200例为研究对象。按随机数字表法将其分为观察组和对照组,每组100例。对照组患者在采用常规留置针退芯法,即见回血后降低穿刺角度将留置针推进0.2 cm,左手持"Y型"接口,右手后撤针芯约0.5 cm,再将留置针全部送入血管后退出全部针芯。观察组患者见回血后右手固定针芯,以针芯为支撑,左手将外套管缓慢送入血管0.5 cm,然后将外套管与针芯一起送入血管,再将针芯全部退出。比较两组患者1次穿刺成功率和留置时间。结果观察组1次穿刺成功率和留置时间分别为95.0%和(5.44±1.81)d,均优于对照组的86.0%和(3.61±1.49)d,差异均有统计学意义(均P<0.05)。结论改良留置针退针芯法可以提高老年住院患者静脉留置针置管成功率,并延长留置时间。

Objective To compare the application effects of two methods of back indwelling needle core in elderly patients with cardiovascular disease. Methods By convenience sampling, 200 cases were selected and randomly divided into observation group and control group, with 100 cases in each group.The control group was applied with routine back indwelling needle core, which reduced the puncture point after see blood return, indwelling needle to promote 0.2 cm, left hand held a ＂Y＂ interface, right hand back the needle core of about 0.5 cm, then fully enter the blood vessels after exit the indwelling needle core. Observation group with the right hand fixed needle core after blood return, supported by needle core, left hand was outside casing Slowly into the blood vessels are 0.5 cm, then put the outer casing with core needle into the blood vessels,then needle core dropped out.The one time puncture success rate and indwelling time of two groups were compared.Results The one time success rate of puncture and indwelling time in observation group were 95 % and （5.44±1.81）d , resepctively, which was better than that of control group of 86% and （3.61±1.49）d, the difference was statistically significant in comparison of the two groups （all P〈0.05）.Conclusion Improved operation method can improve the success rate of venous indwelling needle insertion of elderly hospitalized patients and can extend indwelling time of elderly hospitalized patitents.
