卡前列素氨丁三醇联合低位B-Lynch缝合术治疗难治性前置胎盘性产后出血(PPH)的临床疗效分析

Analysis of Clinical Curative Effect of Romethamine and Low Position b-lynch Suture in Treatment of Lacenta-derived Refractory Postpartum Hemorrhage

目的探讨难治性前置胎盘性PPH应用低位B-Lynch缝合术与卡前列素氨丁三醇联合方案的效果。方法该研究收集2015年10月—2016年10月期间于该院实施治疗的难治性前置胎盘性PPH患者112例作对比研究,以治疗方案为分组依据,应用随机数字表法,将112例患者分为观察组(n=56例)和对照组(n=56例),前组患者应用低位BLynch缝合术+卡前列素氨丁三醇治疗,后组患者应用缩宫素与胎盘缝合术的联合方案,统计分析两组研究数据,探讨不同治疗方案应用于PPH的临床治疗效果。结果观察组总有效率96.43%显著高于对照组75.00%(P<0.05);观察组阴道术后1 d、2 h出血量均显著低于对照组(P<0.05),两组术中出血量对比(P>0.05);观察组不良反应发生率7.14%显著低于对照组23.21%(P<0.05)。结论临床上针对难治性前置胎盘性PPH患者治疗期间应用低位B-Lynch缝合术+卡前列素氨丁三醇的联合方案,可取得确切疗效,有效改善患者术后阴道出血量,且不良反应发生率低于对照组治疗方案,具有较高的临床应用价值。

Objective To discuss the clinical curative effect of romethamine and low position b-lynch suture in treatment of lacenta-derived refractory postpartum hemorrhage.Methods 112 cases of patients with lacenta-derived refractory postpartum hemorrhage treated in our hospital from October 2015 to October 2016 were selected and randomly divided into two groups with 56 cases in each according to different treatment plans,the observation group were treated with romethamine and low position b-lynch suture,while the control group were treated with oxytocin and placenta suture,and the research data were counted and analyzed and the clinical treatment effect of different treatment plans in PPH was discussed.Results The total effective rate in the observation group was obviously higher than that in the control group（96.43% vs 75.00%）（P〈0.05）,and the bleeding amount at 1d and 2h after operation in the observation group were obviously lower than those in the control group（P〈0.05）,and the difference in the intraoperative bleeding amount between the two groups had statistical significance（P〈0.05）,and the incidence rate of adverse reactions in the observation group was obviously lower than that in the control group（7.14% vs 23.21%）,and the difference had statistical significance（P〈0.05）.Conclusion The curative effect of romethamine and low position b-lynch suture in treatment of lacenta-derived refractory postpartum hemorrhage is definite,which can effectively improve the vaginal bleeding amount after operation,and the incidence rate of adverse reactions is lower than that in the control group with higher clinical application value.
