双胎妊娠围产结局的影响因素分析

Analysis of Influence Factors of Perinatal Outcome of Twin Pregnancy

目的探讨影响双胎妊娠围产结局的相关因素。方法对该院产科2014年1月—2015年12月期间分娩的122例双胎妊娠产妇的临床资料进行回顾性分析,将影响双胎妊娠围产结局的相关因素进行归纳总结。结果 122名产妇中,有30.33%(37/122)产妇出现了不良妊娠结局,影响双胎妊娠围产结局的主要因素为绒毛膜性,对双胎预后影响最大,同时各种妊娠期合并疾病、产妇分娩方式的选择也是影响其妊娠结局的重要原因,差异有统计学意义(P<0.05)。结论双胎妊娠属于高危妊娠,临床上应早期确定绒毛性,加强妊娠期及分娩期管理,根据合子性及绒毛性、有无合并症选择分娩方式,以期降低不良围产结局的发生率。

Objective To discuss the related influence factors of perinatal outcome of twin pregnancy.Methods 122 cases of delivery women with twin pregnancy giving birth in our department from January 2014 to December 2015 were selected and the related influence factors of perinatal outcome of twin pregnancy were summarized.Results In the 122 cases of delivery women,30.33%（37/122） had the adverse pregnant outcome,and the main influence factor of perinatal outcome of twin pregnancy was chorionicity,which had the biggest effect on the twin pregnancy prognosis,at the same time,various combined diseases during the pregnancy period and the selection of delivery method of women are also the important causes of pregnant outcome,and the differences had statistical significance（P〈0.05）.Conclusion Twin pregnancy is a high-risk pregnancy,and we should determine the woolliness in the early stage in clinic,enhance the management of pregnant stage and delivery stage and select the delivery method according to the zygosity and woolliness and the complications exist or not in order to reduce the incidence rate of adverse perinatal outcome.
