血管内介入疗法和显微神经外科手术治疗颅内动脉瘤的临床疗效和安全性比较

Comparison of Clinical Curative Effect and Safety of Intravascular Interventional Therapy and Micro-neurosurgery in Treatment of Intracranial Aneurysm

目的分析探讨血管内介入疗法和显微神经外科手术治疗颅内动脉瘤的临床疗效和安全性。方法方便选取2013年7月—2016年7月期间该院收治的145例颅内动脉瘤患者,采用随机数字表法分为观察组(n=72)和对照组(n=73)。对照组患者采用显微神经外科手术治疗,观察组患者采用血管内介入疗法治疗。分析比较两组患者的各项手术相关指标、临床疗效和不良反应发生率。结果观察组患者的术中出血量为(95.34±20.53)mL,小于对照组,手术时间为(89.43±16.77)min、住院时间为(14.08±3.64)d,短于对照组,差异有统计学意义(P<0.05);观察组患者的治疗良好率为60.27%小于对照组,差异有统计学意义(P<0.05);观察组患者的不良反应发生率为13.89%,与对照组患者的不良反应发生率15.00%比较差异无统计学意义(P>0.05)。结论血管内介入疗法治疗颅内动脉瘤对患者损伤较小,显微神经外科手术治疗效果更佳。因此,临床治疗颅内动脉瘤需根据患者的实际生理情况制定治疗方案。

Objective To analyze and discuss the clinical curative effect and safety of intravascular interventional treatment and micro-neurosurgery in treatment of intracranial aneurysm.Methods Convenient selection 145 cases of patients with intracranial aneurysm admitted and treated in our hospital from July 2013 to July 2016 were selected and randomly divided into two groups,the control group were treated with micro-neurosurgery,while the observation group were treated with intravascular interventional therapy,and various operation related indexes,clinical curative effect and incidence rate of adverse reactions of the two groups were compared and analyzed.Results The intraoperative bleeding amount,operation time,length of stay and treatment good rate in the observation group were respectively（95.34±20.53）mL,（89.43±16.77）min,（14.08±3.64）d,60.27%,which were lower than those in the control group,and the differences had statistical significance（P〈0.05）,and the difference in the incidence rate of adverse reaction between the observation group and the control group had no statistical significance（13.89% vs 15.00%）（P〈0.05）.Conclusion The injury of intravascular interventional therapy in treatment of intracranial aneurysm is small,and the treatment effect of micro-neurosurgery is better,therefore,the treatment plan should be made according to the practical physical condition of patients with intracranial aneurysm in the clinical treatment.
