前列地尔注射液联合奥曲肽治疗急性重症胰腺炎的疗效及对患者血清TNF-α、IL-6、IL-18、闭合蛋白水平的影响

目的 探讨前列地尔注射液联合奥曲肽用于急性重症胰腺炎的疗效及对血清肿瘤坏死因子-α（TNF-α）、白细胞介素-6（IL-6）、白细胞介素-18（IL-18）、闭合蛋白水平影响。方法 选取2013年9月~2017年2月于绍兴文理学院附属医院就诊的68例急性重症胰腺炎患者,按治疗方法分组,每组34例。对照组采用奥曲肽治疗,研究组在对照组基础上加用前列地尔注射液治疗,2组均持续用药5 d。比较2组临床疗效,临床表现缓解时间,TNF-α,IL-6,IL-18,闭合蛋白和不良反应发生情况。结果 研究组治疗总有效率为97.07%,显著高于对照组79.41%（P〈0.05）。研究组肠鸣音、腹痛、体温及血清淀粉酶缓解时间均短于对照组（P〈0.05）。研究组TNF-α、IL-6、IL-18水平显著低于对照组（P〈0.05）,研究组闭合蛋白高于对照组（P〈0.05）。2组不良反应发生率比较差异无统计学意义。结论 前列地尔注射液联合奥曲肽用于急性重症胰腺炎的疗效确切,改善血清TNF-α、IL-6、IL-18、闭合蛋白水平。

Objective To research the efficacy of alprostadil injection combined the octreotide for acute severe pancreatitis and influence on the serum levels of tumor necrosis factor-α（ TNF-α） , interleukin-6, interleukin-18, occludin. Method 6 8 c a s e s of patients w i t h a c u t e s e v e r e pancreatitis from September 2013 to February 2017 in our hospital, according to the treatment method group, 34 cases in each group, control group treatmented by octreotide, the research group based on the control group treatmented by alprostadil injection, both groups was treated for five days. Clinical curative effect, alleviate clinical symptoms time, TNF-a, IL-6, IL-18, occludin, and adverse reactions occur was compared between two groups. Results T h e total effective rate of research group was higher than the control group （97.07% vs. 79. 41% , P 〈 0. 0 5 ） . B o w e l s o u n d s , s t o m a c h a c h e , b o d y temperature and serum amylase remission time of research group was shorter than control group （P 〈0. 05）. TNF-α, IL-6, IL-18 of research group was lower than the control group （ P 〈 0. 05 ） , the occludin of research group was higher than the control group （P 〈 0. 05 ）. The adverse reactions was no differences between the two groups. Conclusion The e x a c t effect o f alprostadil injection combined the octreotide for a c u t e s e v e r e p a n c r e a t i t i s , improve serum levels of TNF-α, IL-6, IL-18, occludin.
