摘要
目的 探究匹多莫德与抗病毒药物联合对传染性单核细胞增多症患儿免疫功能及心肌酶谱影响。方法 选取温州市儿童医院呼吸科诊断为传染性单核细胞增多症患儿94例,按随机数字表法分为对照组与研究组,每组47例,对照组予以常规抗病毒治疗;研究组在常规抗病毒治疗基础上予以匹多莫德治疗,2周为一疗程,治疗前后采血测定免疫功能、细胞因子及心肌酶谱等指标,观察并记录临床症状、体征缓解状况,同时对比临床疗效及并发症。结果 与治疗前比较,治疗后患儿血清CD4+、CD4+/CD8+和Ig A、Ig G含量升高,CD8+含量减低,血清TNF-α、IL-6含量降低,血清AST、LDH、CK、CK-MB含量降低(P〈0.05);与对照组比较,研究组治疗后血清CD4+、CD4+/CD8+和Ig A、Ig G含量较高,CD8+含量较低,血清TNF-α、IL-6含量较低,血清AST、LDH、CK、CK-MB含量较低(P〈0.05);研究组热退、咽峡炎消退、淋巴结缩小、肝脾缩小及住院时间均低于对照组(P〈0.05);对照组有效率74.47%低于研究组有效率91.49%(P〈0.05)。结论 匹多莫德与抗病毒药物联合治疗传染性单核细胞增多症疗效确切,可提高免疫功能、降低心肌酶谱及炎症指标。
Objective To investigate the effect of pidotimod combined with antiviral drugs on immunologic function and myocardial enzyme spectrum in children with infectious mononucleosis. Methods 94 c a s e s of c h i l d r e n w i t h infectious m o n o n u c l e o s i s s e l e c t e d in Wenzhou C h i l d r e n ^ hospital were randomly divided into control group and research group, 47 cases in each group. The control group were given conventional antiviral treatment ; O n his basis, the research group were given pidotimod, 2 weeks for one period of treatment. Before and after treatment, immune function, cell factors and myocardial enzyme spectrum were tested, the clinical symptoms, signs, the curative effect and complications were observed and compared. Results Compared with before treatment, after treatment, serum CD4 + , CD4+/CD8+ IgA and IgG increased, CD8 + decreased, TNF alpha, IL- 6, AST, LDH, CK, CK - MB decreased (P 〈 0. 05). Compared with the control group, in the research group, CD4 + , CD4 + /CD8 + , IgA and IgG content s higher, CD8 + content is lower, the TNF alpha, IL - 6, AST, LDH, CK, CK-MB content is lower (P 〈0. 05). The time of antipyretic, angina fade, narrow lymph nodes, liver and spleen shrinks and length of hospital stay in the research group were shorter than that in the control group ( P 〈 0. 05 ).The effective rate was 74. 47% in the control group, lower than 91. 49% of research group ( P 〈 0. 0 5 ). Conclusion T h e c u rative is e x a c t that pidotimod combined with antiviral drugs on the treatment of infectious mononucleosis. It could improve immune function, reduce myocardial enzyme spectrum and inflammation.
