三仁汤加减治疗湿热伤阴型干眼症临床观察

Clinical Observation of Modified Sanren Decoction in the Treatment of Xerophthalmia Differentiated as Damp Heat and Yin Damage

目的探讨三仁汤加减方治疗湿热伤阴型干眼症的临床疗效。方法 60例湿热伤阴型干眼症患者按随机数字表法分为治疗组和对照组各30例,对照组给予0.1%右旋糖酐羟丙甲纤维素滴眼液滴眼,治疗组在对照组用药的基础上另给予三仁汤加减方口服。两组均治疗1个月后比较疗效,观察治疗前后两组患者基础泪液分泌试验(SIt)、泪膜破裂时间(BUT)、角膜荧光素染色(FL)评分改变情况。结果 2组连续用药1个月后,BUT、SIt、FL评分好于治疗前,治疗组各项指标变化明显优于对照组(P<0.05)。治疗组总有效率为90.96%,对照组总有效率68.75%,两组比较差异有统计学意义(P<0.05)。结论三仁汤在改善湿热伤阴型干眼患者的自觉症状,促进患者自身泪液分泌,延长泪膜破裂时间方面,疗效较单纯局部使用人工泪液更确切,为临床治疗这一疾病提供新的有效方剂。

Objective To discuss the clinical therapeutic effects of modified sanren decoction on xerophthalmia differentiated as damp heat and yin damage. Methods Sixty patients of xerophthalmia differentiated as damp heat and yin damage were divided into a treatment group and a control group, 30 cases in each one according to random number table. In the control group ,0.1% dextran and hypromellose eye drops were used. In the treatment group, on the basis of the treatment as the control group, the modified sanren de- coction was added for oral administration. After 1 - month treatment, the therapeutic effects were compared in the two groups;the scores of SIT, BUT and FL were compared before and after treatment in the two groups. Results After 1 - month treatment in two groups, BUT, SIT and FL scores were better than those before treatment in the two groups. The changes in the treatment group were apparently better than those in the control group（P 〈0.05）. The total effective rate was 90.96% in the treatment group and was 68.75% in the control group, indicating the significant differences in comparison （ P 〈 0.05 ）. Conclusion The therapeutic effects of sanren decoction are more definite as compared with simple local use of artificial tears on relieving the subjective symptoms, promoting tear secretion and prolonging breakup time of tear film in the patients with xerophthalmia differentiated as damp heat and yin damage, which provides a new effective formula for the clinical treatment of this disease.
