后外侧入路手术在三踝关节骨折患者中的应用

Application of Operation through the Posterior-lateral Approach in Patients with Trimalleolar Fractures

目的探讨后外侧入路手术在三踝关节骨折患者中的应用。方法方便选取该院2013年6月—2016年10月收治的118例三踝关节骨折患者,随机分为两组。治疗组59例采用后外侧入路手术治疗,而对照组59例采用前入路手术治疗。比较两组患者临床疗效、手术指标、治疗满意度及生活质量的差异。结果治疗组临床优良率为94.9%稍高于对照组91.5%,治疗满意率为93.2%稍高于对照组89.8%,比较差异无统计学意义(P>0.05);治疗组手术时间、骨折愈合时间高于对照组,比较差异有统计学意义(P<0.05);手术后2月治疗组生活质量评分显著升高(P<0.05)。结论后外侧入路手术在三踝关节骨折效果佳,其具有较高的临床优良率,生活质量也得到改善,治疗满意度也较高,但其较前入路恢复稍慢,值得临床选择。

Objective To study the application of operation through the posterior-lateral approach in patients with trimalleolar fractures.Methods 118 cases of patients with trimalleolar fractures admitted and treated in our hospital from June 2013 to October 2016 were conveniently selected and divided into two groups with 59 cases in each,the treatment group were treated with the operation through the Posterior-lateral Approach,while the control group were treated with operation through the anterior approach,and the clinical curative effect,operative index,treatment satisfactory degree and quality of life were compared between the two groups.Results The clinical excellent and good rate in the treatment group was slightly higher than that in the control group（94.9% vs 91.5%）,and the treatment satisfactory rate was slightly higher than that in the control group,（93.2%vs 89.8%）,and the difference had no statistical significance（P〈0.05）,and the operation time and fracture healing time in the treatment group were higher than those in the control group,and the differences had statistical significance（P〈0.05）,after 2-month operation,the quality of life score obviously increased in the treatment group（P〈0.05）.Conclusion The effect of operation posterior-lateral approach in patients with trimalleolar fractures is good with higher clinical excellent and good rate and higher treatment satisfactory degree,which can improve the quality of life,but the recovery is slower than that of operation through the anterior approach,and it is worth clinical selection.
