摘要
目的探讨带血管骨移植在四肢骨肿瘤治疗中的临床应用价值。方法选取90例四肢骨肿瘤患者,按住院单双号分为2组,对照组(44例)给予瘤段切除后无血管骨移植,观察组(46例)给予瘤段切除后带血管骨移植。通过观察并记录患者术后肢体功能情况、骨愈合情况及随访期间并发症发生率,评价带血管骨移植在四肢骨肿瘤治疗中的临床应用价值。结果术后12个月,观察组肢体功能总的优良率明显高于对照组(P<0.05),但2组骨愈合率差异无统计学意义(P>0.05)。观察组患者平均骨愈合时间明显短于对照组(P<0.05)。随访期间,观察组并发症发生率4.3%,对照组并发症发生率25.0%,观察组术后并发症发生率明显低于对照组(P<0.05);2组均未发生伤口感染、下肢深静脉血栓、内固定物松动等并发症。结论带血管骨移植可以明显提高四肢骨肿瘤的治疗效果,骨愈合时间短,术后并发症较少,值得临床推广使用。
Objective To study the clinical application value of vascularized bone graft in the treatment of bone tumor of extremities. Methods 90 patients with bone tumor of extremities were selected, they were divided into 2 groups according to oddeven number of hospitalization. The control group (44 cases) was given non vascularized bone graft. The observation group (46 cases) was given vascularized bone graft. The clinical application value of vascularized bone graft in the treatment of bone tumor of extremities was evaluated by function of the limbs, bone heal and complication rate during follow-up period. Results After 12 months surgery, the excellent rate of the observation group was higher than that of the control group ( P 〈 0.05 ). There were no statistical significance on bone healing rate between the 2 group ( P 〉 0.05 ). The average bone healing time of the observation group was significantly shorter than of the control group ( P 〈 0.05 ). During follow-up, the complication rate of the observation group was 4.3 %. The complication rate of the control group was 25.0 %. The complication rate of the observation group was lower than that of the control group ( P 〈 0.05 ). The wound infection, deep venous thrombosis and internal fixation loosening were not occurred in the 2 groups during follow-up. Conclusion Vascularized bone graft could significantly improve the therapeutic effect of bone tumor of extremities. It could make bone healing time shorter and complication less. It is worthy of clinical application.
