湖北十堰地区女性宫颈人乳头状瘤病毒感染与亚型分布 被引量:10

摘要 目的研究湖北十堰地区女性宫颈脱落细胞人乳头状瘤病毒(HPV)感染和亚型分布情况。方法收集2015年12月~2016年12月在湖北医药学院附属东风医院接受HPV检测的8543名女性为研究对象,采用PCR基因扩增和免疫杂交技术,进行21种HPV亚型的基因分型检测。结果总样本数8543例,共检查HPV阳性标本783例(高危604例,低危179例),感染率为9.17%;其中,单一亚型感染718例,占91.70%,多重感染65例,占8.30%;前五位单一高危险型分别为52型(114例)、16型(86例)、58型(84例)、53型(60例)、39型(40例),其阳性率分别为14.56%、10.98%、10.73%、7.66%、5.11%。≥55岁年龄组感染率最高为17.07%,<25岁年龄组阳性率为15.46%,25~<35岁年龄组感染率为9.20%,35~<45岁和45~<55岁年龄组感染率分别为7.76%和9.90%。结论湖北十堰地区女性HPV感染率较居中,以单一高危感染为主,HPV52、16、58型为前三大常见感染亚型,且各年龄段HPV感染率构成比呈现"U"型走势。 Objective To investigate the infection and genotype distribution of human papilloma virus (HPV) in female cervical exfoliative cells in Shiyan, Hubei province. Methods Samples of cervical exfoliated cells were collected from 8543 female in Dongfeng Hospital Affiliated to Hubei University of Medicine from December 2015 to December 2016 as study objects. Twenty-one HPV subtypes were examined by PCR gene amplification and immune hybrid technology. Results In the total sample of 8543, HPV positive was detected in 783 cases (604 cases of high risk HPV, and 179 cases of low risk HPV), total infection rate of HPV was 9.17%. Among these positive samples, the single-subtype infec- tion rate was 91.70% (718 cases), and the multiple-subtype infection rate was 8.30% (65 cases). HPV-52 (114 cases, 14.56%), HPV-16 (86 cases, 10.98%), HPV-58 (84 cases, 10.73%), HPV-53 (60 cases, 7.66%) and HPV-39 (40 cases, 5.11%) were the top five prevalent subtypes. The highest HPV positive rate (17.07%) was found in older than 55 years age group. The rate of younger than 25 group was 15.46%, the rates of 25-〈35 years, 35-〈45 years and 45-〈55 years age group was 9.20%, 7.76% and 9.90% respectively2 Conclusion The female HPV infection rate in Shiyan Area is median, and the single and high-risk subtype infection accounts for a considerable proportion. HPV-52, HPV-16 and HPV-58 are the top three prevalent subtypes, and the distribution of HPV infection rate in each age group pre- sented "U" shaped trend.
