首次剖腹产切口的选择对二次剖腹产手术的影响

The Effect of the Choice With First Cesarean Section Incision on Second Cesarean Section

目的研究首次剖腹产切口的选择对二次剖腹产手术的影响。方法选择进行二次剖腹产手术的产妇84例,依据其首次剖腹产的切口方式分为甲组(腹壁横切口)和乙组(腹壁纵切口),比较两组产妇手术的各项指标。结果乙组产妇的开腹时间、胎儿娩出时间以及总手术时间均短于甲组,术中出血量少于甲组(P<0.01);乙组产妇腹腔粘连程度轻于甲组(P<0.05)。结论在进行首次剖腹产时,腹腔切口的选择应慎重,有再次妊娠打算者,应尽量选择腹部纵行切口,降低二次剖腹产的危险性。

Objective To study the effect of the choice with first cesarean section incision on second cesarean section. Methods 84 cases of maternal second cesarean section were selected and divided into group A(abdominal transverse incision) and group B(abdominal longitudinal incision) according to the incision of the first cesarean section. The indexes of maternal surgery were compared between the two groups. Results Group B were open time and delivery time and total operation time were shorter in group A, the amount of intraoperative bleeding less than group A(P < 0.01). Group B maternal abdominal adhesions lighter than group A(P < 0.05). Conclusion In the first cesarean section, the choice of abdominal incision should be careful, there are re-pregnancy plan, should try to choose abdominal longitudinal incision, reduce the risk of secondary cesarean section.
