老年慢性病患者健康管理干预的研究

The Research of the Health Management Intervention on the Elderly Patients With Chronic Disease

目的研究健康管理干预对老年慢性病的影响。方法随机抽取某社区208位老年慢性病患者为研究对象,对其进行两个月的健康管理干预,分析其管理前后体检指标的变化和健康管理的效果。结果健康管理后老年患者血压、血糖、血脂和心率各项指标正常率均有提高(P<0.05),健康管理后患者平均分值高于健康管理前平均分值(P<0.05)。结论健康管理对老年慢性病患者病情控制产生良性的影响,医疗机构应该针对老年慢性病患者积极开展健康管理工作。

Objective Researching the influence on the senile chronic diseases by the health management intervention. Methords Random drawing 208 elderly patients with chronic diseases from a certain community as a survey sample. Carrying out two months health management interventions, analyzing the change of indexes and evaluating the effect of health management. Results After health management interventions, the normal proportion of blood pressure, blood fat and heart rate increase （P 〈 0.05）, the average score after health management interventions is higher than before （P 〈 0.05）. Conclusion The health management interventions have an benignant effect on elderly patients with chronic diseases.Aiming at elderly patients with chronic diseases, the medical institutions should develop the health management actively .
