帕瑞昔布钠对肺移植缺血再灌注损伤的影响 被引量:3

摘要 目的研究帕瑞昔布钠对肺移植缺血再灌注损伤的影响。方法将移植成功后的16只Wistar大鼠随机分为对照组和治疗组(n=8)。肺移植后对照组给予生理盐水;治疗组再灌注后给予帕瑞昔布钠静脉注射。24 h后处死受体,记录基础状态和再灌注后动脉氧分压。测量组织匀浆中TNF-α、IL-1β、IL-10和弹性蛋白酶的浓度,测量移植肺组织湿/干重比,并取部分组织行组织学分析。结果再灌注24 h后,治疗组的PaO_2/FiO_2高于对照组(P<0.05);对照组TNF-α、IL-1β和弹性蛋白酶浓度高于治疗组(P<0.05),IL-10低于治疗组(P<0.05);对照组移植肺组织湿/干重比高于治疗组(P<0.05);组织学分析提示与对照组比较,治疗组肺组织中出血、水肿明显减轻,巨噬细胞和中性粒细胞浸润减少。结论帕瑞昔布钠对肺移植缺血再灌注损伤有抑制作用,能够改善移植肺组织换气功能和肺毛细血管通透性,减少TNF-α、IL-1β和弹性蛋白酶,促进Il-10生成。 Objective To investigate the effect of the parecoxih on the ischemia reperfusion in- jury in rats receiving lung transplantation. Methods Sixteen Wistar rats were randomized in control group and therapeutic group after the transplantation, The recipients in control group were injected with saline and the therapeutic group were injected with parecoxib after reperfu- sion. All the groups were observed for 24 hours after the reperfusion. The PaOJFiO2 at base- line and 24 hours after the reperfusion were recorded. The TNF- α, IL-1β, IL-10 and elastase in the homogenate were measured. The wet to dry weight ratio was calculated. Moreover, part of transplantated lung was made histological analysis. Results The PaO2/FiO2 in therapeutic group were significantly higher than that in control group for 24 hours after the reperfusion (P 〈0. 05). The TNF- α, IL-1β and elastase concentrations in control group were higher than those in therapeutic group (P 〈0. 05) , and IL-10 was lower than the therapeutic group (P 〈 0. 05 ). The wet to dry weight ratio in control group was significantly higher than that in therapeutic group (P 〈 0. 05 ). The histological analysis showed that the haemorrhage and edema were signigicantly ameolated in therapeutic group. Conclusion The parecoxib could reduce the lung ischemia reperfusion injury and improve the gas exchange and pulmonary alWolocapillary permeability of transplantated lung via decrease of pro-inflammatory factors including TNF- α, IL-1β and elastase and increase of anti-inflammatory cytokine IL-10.
