生物反馈联合电刺激和阴道哑铃训练治疗产后妇女盆底功能障碍的效果比较

Effect comparison of biofeedback combined with electrical stimulation therapy and vaginal dumbbell training in treatment of the postpartum women with pelvic floor dysfunction

目的探讨生物反馈联合电刺激和阴道哑铃训练在产后妇女盆底功能障碍中的治疗效果差异。方法选择2015年11月~2016年11月我院收治的80例产后盆底功能障碍性疾病患者作为研究对象,根据治疗方法分为观察组和对照组,各40例。其中观察组予以生物反馈联合电刺激治疗,对照组予以阴道哑铃训练治疗。比较两组患者治疗前后盆底肌肉肌力情况及功能情况。结果治疗前两组患者盆底Ⅰ、Ⅱ类肌的肌力均为Ⅱ、Ⅲ级,比较差异无统计学意义(P>0.05);治疗后,观察组患者盆底肌力明显提高,其中Ⅰ类肌Ⅳ、Ⅴ级肌力的患者占比分别为22.5%、72.5%,明显高于对照组(5.0%、0%),Ⅱ类肌Ⅳ、Ⅴ级肌力的患者比例为22.5%、70.0%,明显高于对照组(2.5%、2.5%),差异有统计学意义(P<0.05);治疗后观察组患者的腰腹酸痛、阴道干涩、阴道松弛及尿漏、尿频发生率明显低于对照组,差异有统计学意义(P<0.05)。结论生物反馈联合电刺激治疗产后盆底功能障碍性疾病具有更好的效果,显著提高患者的盆底肌力,改善患者的症状,值得临床推广。

Objective To study the difference of biofeedback combined with electrical stimulation therapy and vaginal dumbbell training in treatment of the postpartum women with pelvic floor dysfunction.Methods 80 patients with pelvic floor dysfunction treated in our hospital from November 2015 to November 2016 were selected as the research objects, according to the treatment methods,who were divided into the observation group （n=40） and the control group （n=40）;the patients in the observation group were given biofeedback combined with electrical stimulation therapy,and patients in the control were given vaginal dumbbell training;the pelvic floor muscle strength and function of two groups before and after treatment were compared.Results Before treatment,the difference of the pelvic floor muscle strength between two groups was not statistically significant （P〉0.05）;after treatment,thepelvic floor muscle strength of the patients in the ob- servation group increased significantly,and there were 22.5%,72.5% cases whose strength of muscle fiber I reached to grade IV, V in the observation group,which were significantly more than 5.0%,0%eases in the control group,and there were 22.5%,70.0%cases whose strength of muscle fiber II reached to grade IVandV in the observation group,which were significantly more than 2.5%,2.5% cases in the control group,and differences were statistically significant （P〈0.05）;after treatment,thesymptpm occurrence of lumber abdomen pain,vaginal dryness,vaginal flabbiness and urine leakage in the ob- servation group were significantly less than those in the control group,and the differences were statistically significant （P〈0.05）.Conclusion The biofeedback combined with electrical stimulation in treatment of pelvic floqr functional disor- der has the better effect,which can significantly improve pelvic floor muscle strength,and relive the symptoms of patient, so it is worth of clinical promotion.
