(11)~C-CFT脑多巴胺转运体PET显像在不同类型帕金森病患者病情评估中的应用价值 被引量:7

摘要 目的探讨^(11)C-CFT脑多巴胺转运体(DAT)正电子发射断层显像(positron emission tomography,PET)对不同临床HY分期、不同年龄亚型及不同症状亚型帕金森病患者(PD)的临床应用价值。方法选取临床确诊62例PD患者,根据临床Hoehn-Yahr PD分期(HY分期)对PD患者进行早晚期分组,以及不同年龄亚型即早发型PD(Early onset PD,EOPD)与晚发型PD(Late onsetPD,LOPD)分组,不同症状亚型即震颤型PD(TDT)与无震颤型PD(nTDT)分组,同时随机选取30例新疆医科大学第一附属医院健康体检者作为对照组,分别行^(11)C-CFT脑多巴胺转运体(dopamine transportor,DAT)PET显像,通过勾画感兴趣区(region of interest techniques,ROI)比较PD组与对照组之间双侧基底节区^(11)C-CFT摄取差异,评估^(11)C-CFT PET显像对于临床HY早晚分期、不同年龄亚型及不同症状亚型PD患者中的应用价值。结果 HY早、晚期PD组与对照组之间双侧基底节区尾状核、壳核前、中、后部之间^(11)C-CFT摄取值差异有统计学意义(P<0.05);早、晚期PD组之间基底节区壳核^(11)C-CFT摄取减低均以重侧对侧基底节^(11)C-CFT摄取减低明显,并以壳核后部减低为著。EOPD与LOPD组之间、TDT与nTDT组之间双侧基底节区尾状核、壳核前、中、后部之间^(11)C-CFT摄取值差异无统计学意义(P>0.05)。结论 ^(11) C-CFT PET显像对于HY早期与HY晚期具有良好的评估价值,但在不同年龄亚型及不同震颤亚型中^(11)C-CFT PET显像鉴别意义不大。 Objective To explore the uptake of ^(11)C-CFT brain dopamine transporter(DAT)PET imaging in brain basal ganglia in patients with Parkinson′s diseaseand toanalyze the characteristics and application value of brain dopamine transporter imaging in different clinical HY stages and subtypes of PD patients.Methods 62 cases of patients clinically diagnosed with PD were selected,Classification was performed according to the clinical HY stages,age and symptoms.Meanwhile,the other 30 cases of healthy persons were used as control group.^(11)C-CFT brain dopamine transporter(DAT)PET imaging was performed in the two groups and the difference of ^(11)C-CFT uptake in basal ganglia region between the Parkinson group and healthy group was compared by outlining the region of interest(ROI).Analysis of ^(11)C-CFT uptake in basal ganglia in patients with different clinical stages,different age and symptom subtype PD.Evaluation of the value of ^(11) C-CFT brain dopamine transporter PET imaging in patients with Parkinson.ResultsThere was significant difference in terms of the uptake value of ^(11)C-CFT in the caudate nucleus,putamen,middle and posterior part of the bilateral basal ganglia between HY early PD group and healthy HY late PD group(P 0.05).In the early and late stage PD patients,uptake value of^(11)C-CFT on the contralateral basal ganglia putamen DAT distribute was presented to bedecreased,posterior putamen DAT distribute was shown to be decreased more prominently.There was no significant difference of uptake value of ^(11)CCFT in different subgroups of EOPD and LOPD,different subtypes of motor symptoms that tremor type(TDT)and non tremor type(nTDT)PD group(P 0.05).Conclusion ^(11)C-CFT PET imaging was displayed to have a good evaluation value for early and late HY,but there was no significant difference in different age subtypes and different tremor subtypes.
