白芍总苷联合沙利度胺治疗强直性脊柱炎的应用价值及对CRP、ESR、TNF-α的影响

Application value of paeony glmcosides and thalidomide in the treatment of ankylosing spondylitis and the effects on CRP,ESR and TNF-α

目的探讨白芍总苷联合沙利度胺治疗强直性脊柱炎(ankylosing spondylitis,AS)的应用价值及对CRP、ESR、TNF-α的影响。方法将150例AS患者随机分为两组,各75例。观察组给予白芍总苷联合沙利度胺治疗,对照组仅采用沙利度胺治疗。对比两组临床疗效、腰背痛指数、外周关节痛指数、晨僵时间、Schober试验、血沉(ESR)、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)及不良反应发生率。结果观察组临床疗效高于对照组(P<0.01);观察组治疗后腰背痛指数、外周关节痛指数、晨僵时间、Schober试验、ESR、CRP及TNF-α均较治疗前显著改善,且观察组改善程度均优于对照组(P<0.01);观察组不良反应发生率16.0%,低于对照组的36.0%(P<0.01)。结论白芍总苷联合沙利度胺治疗AS效果显著,能有效改善CRP、ESR及TNF-α指标,且不良反应少,值得推广。

Objective To investigate the application value of paeony glmcosides and thalidomide in the treatment of ankylosing spondylitis （AS） and the effects on CRP, ESR and TNF-α. Methods One hundred and fifty cases of patients with AS were divided into the observation group and the control group randomly（ n = 75 in each group）. The observation group was treated with paeony glmcasides and thalidomide, and the control group only treated with thalidomide. The clinical efficacy,low baelc pain index, peripheral joint pain index, time of morning stiffness, schober test, ESR, CRP, TNF-α and incidence of adverse reactions were compared. Results The total effective rate of the observation group （94. 7% ） was significantly higher than that （69. 3% ） of the contred group （ P 〈 0. 01 ）. The improvement degree of low back pain index, peripheral joint pain index, time of morning stiffness, Schober test, ESR, CRP and TNF-α of the observation group were significantly better than those control group （P 〈 0. 01 ）. The incidence rate of adverse vation group（ 16.0% ） was significantly lower than that （36.0%）of the control group（ P〈0.01）.Conclusion Paeony ghncosides and thalidomide in the treatment of AS can effectively improve the CRP, ESR and TNF-α indiea ,and with less adverse reactions, and is worth promoting.
