2011--2015年某军队医院住院患者医疗费用变化分析

Analysis on Trend and Correlation Degree of Hospitalization Expenses in a Hospital of PLA During 2011 to 2015

目的了解2011—2015年某部队医院住院患者医疗费用的内部影响因素及变动情况,为住院费用结构调整、有效控制医疗费用提供理论依据。方法采用新灰色关联和结构变动度分析法,分析5年间住院医疗费用各因素之间的关联程度和结构变动。结果 5年间住院患者的各项费用中,药品费用的关联度最大(1.000),其次是手术费(0.831);手术费的结构变动贡献率最大(50.0%),其次是药品费(18.17%),两者的累计贡献率为68.17%。结论药品费和手术费是影响住院费用的主要因素,合理支配药品费用和降低不必要的手术相关费用能够减轻患者的住院费用负担。

Objective To understand the internal influence factors and the changes of hospitalization expenses in a hospital of PLA during 2011 to 2015, in order to provide theoretical evidence for structure adjustment and effective control of medical expense. Methods Grey correlation analysis and degree of structure variation analysis were used to quantitatively evaluate the correlation and structure variation of the hospitalization expense factors during 2011 to 2015. Results Among various hospitalization expenses in 5 years, the medicine expense was the most closely linked to the hospitalization expense （ 1. 000 ）. Second on the list was the operation expenses （0. 831 ）. The operation expenses contributed mostly to the degree of structure variation（50.0% ）, and second on the list was the medicine expenses（ 18.17% ）. The accumulated contribution of this two expenses was 68.17%. Conclusion the medicine expenses and the operation expenses are two major influencing factors for hospital costs controlling. Reasonable control of medicine expenses and reduction of unnecessary operation expenses could decrease the hospitalization costs burden of the patients.
