甲状腺功能正常的2型糖尿病甲状腺激素与嘌呤代谢和体重的相关性 被引量:5

The Relationship between Thyroid Hormone and Purine Metabolism and Body Weight in Patients with Type II Diabetes and Normal Thyroid Function

摘要 目的探讨甲状腺功能正常的2型糖尿病患者甲状腺激素与尿酸(UA)和体重指数(BMI)的关系.方法选取确诊的甲功正常的2型糖尿病患者313例,入院后测BMI、空腹血糖(FBG)、血脂、甲状腺激素和UA等代谢指标,分析甲状腺激素和BMI及UA的相关性.结果 (1)根据性别分为2组,男性FT3、FT4、UA显著高于女性(P<0.01),女性TSH、SBP、HDL-C显著高于男性(P<0.01);(2)按BMI水平分为3组,超重及肥胖组促甲状腺激素(TSH)、三碘甲腺原氨酸(TT3)、游离三碘甲状腺原氨酸(FT3)、UA、FBG均高于体重正常组(P<0.05);(3)按TSH水平分为2组,TSH>2.5μIU/L组血清尿酸、TT3、FT3、空腹胰岛素高于TSH<2.5μIU/L组(P<0.05);(4)按UA水平分为2组,男性高尿酸组TSH及FT3高于正常尿酸组,P<0.05;女性高尿酸组TSH高于正常尿酸组(P<0.05).结论甲状腺激素对评估2型糖尿病患者的体重和尿酸有重要的临床意义. Objective To investigate the relationship between thyroid hormone and uric acid (UA) and body mass index (BMI) in patients with type I! diabetes and normal thyroid function. Methods Total of 313 pa- tients with type II diabetes and normal thyroid function were selected. BMI, fasting blood glucose (FBG) , the metabolism of blood lipid, thyroid hormones and UA indicators were examined and the correlations of thyroid hor- mone, BMI and UA were analyzed. Results (1) The patients were divided into two groups according to gender, and FT3, FT4, and UA of male were found to be significantly higher than those in female (P〈O.O1) . TSH, SBP, HDL-C in female were significantly higher than those in male (P〈O.O1) ; (2) The patients were divided into three groups according to BMI level. Thyroid-stimulating hormone (TSH) , three iodine armour gland original glycine (TT3), free three iodine thyroid glycine (FT3), UA, and FBG in overweight and obesity groups were found to be higher than those in normal weight group (P 〈 0.05) ; (3) The patients were divided into two groups according to the TSH level. Serum uric acid, T-F3, FT3, fasting insulin in the group with TSH above 2.5 ulU/Lwere found to be higher than those in the group with TSH under 2.5 ulU/L (P 〈 0.05) ; (4) Patients were divided into two groups according to the UA level. TSH, FT3 in male with high uric acid were found to be higher than those in male with normal uric acid (P 〈 0.05) ; TSH was in female with high uric acid was found to be higher than that in female with normal uric acid (P 〈 0.05) . Oonclusion Thyroid hormone in patients with type II diabetes can be used to assess the body weight and uric acid, which is of great clinical importance.
