血清胱抑素C、β_2-微球蛋白和尿碱性磷酸酶在糖尿病肾病诊断中的临床价值 被引量:17

Clinical value of serum cystatin C,beta 2-microglobulin and urine alkaline phosphatase in the diagnosis of diabetic nephropathy

摘要 目的探讨血清胱抑素C(Cys-C)、β_2-微球蛋白(β_2-MG)和尿碱性磷酸酶(U-ALP)在糖尿病肾病中的诊断价值。方法将528例糖尿病患者分成3个组即肾功能无损伤组、肾功能轻度损伤组和肾功能重度损伤组,与对照组85例健康体检者分别比较血生化指标及尿生化指标检测结果,并进行统计学分析。结果糖尿病患者肾功能轻度损伤组和肾功能重度损伤组血清Cys-C、β_2-MG和U-ALP检测结果均高于肾功能无损伤组和对照组,差异有统计学意义(P<0.05);糖尿病患者肾功能无损伤组检测结果与对照组比较,差异无统计学意义(P>0.05);Cys-C、β_2-MG和U-ALP联合检测阳性检出率高于单个项目阳性检出率,差异有统计学意义(P<0.05)。结论血清Cys-C、β_2-MG和U-ALP可用于糖尿病肾病的临床诊断,联合检测血清Cys-C、β_2-MG和U-ALP可以提高阳性检出率。 Objective To explore the value of serum cystatin C ( Cys - C ), beta 2 - microglobulin ( β2 - MG) and urine alka-line phosphatase( U - ALP) in the diagnosis of diabetic nephropathy. Methods 528 cases of diabetic patients were divided into non renal damage group, mild renal injury group and severe renal injury group; blood biochemical indexes and urine biochem- ical indexes of the above 3 groups were compared with the control group for statistical analysis. Results The detection results of Cys - C, U - ALP and β2 - MG in diabetic patients of mild injury group and severe damage group were higher than those in non injury group and control group, with the differences statistically significant( P 〈 0.05 ) ; and there was no statistical significance in the differences between non injury group and the control group(P〉0.05 ) ; the positive rate of combined detection of Cys- C, β2 - MG and U - ALP was higher than that of any single detection, with difference statistically significant( P 〈 0.05 ). Conclusion Serum Cys - C, β2 - MG and U - ALP can be used in the clinical diagnosis of diabetic nephropathy, and combined detection of them can improve the positive detection rate.
