中药内服、外洗联合左氧氟沙星治疗老年盆腔炎随机平行对照研究

Randomized Controlled Study of Oral Administration of Traditional Chinese Medicine combined with External Washing combined with Levofloxacin in the Treatment of Elderly Patients with Pelvic Inflammatory Disease

[目的]观察中药内服、外洗联合左氧氟沙星治疗老年盆腔炎疗效。[方法]使用随机平行对照,将90例患者按就诊顺序编号方法简单随机分为两组。对照组45例左氧氟沙星0.3g+5%葡萄糖250mL,静滴,2次/d,滴注≥2h。治疗组45例中药内服:巴戟天30g,补骨脂、菟丝子各25g,肉桂20g,附子5g,杜仲、白术各20g,山药15g,人参20g,水煎300mL,早晚温服;中药外洗:消炎汤(金银花、蒲公英、败酱草、紫花地丁各30g,香附、丹参各15g、红花6g,延胡索30g,苦参20g),熬制2h,留置300mL,取100mL兑温水至400mL外洗,2次/d,30min/次,注意防止烫伤;西药治疗同对照组。连续治疗10d为1疗程。观测临床症状、血清炎症因子(TNF-α、IL-2、IL-4、IL-10)、不良反应。治疗1疗程(10d),判定疗效。[结果]治疗组痊愈5例,显效18例,有效20例,无效2例,总有效率95.56%;对照组痊愈2例,显效16例,有效19例,无效8例,总有效率82.22%;治疗组疗效优于对照组(P<0.05)。TNF-α、IL-2、IL-4、IL-10两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]中药内服、外洗联合左氧氟沙星治疗老年盆腔炎,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of traditional Chinese medicine oral administration and external washing combined with levofloxacin in the treatment of elderly patients with pelvic inflammatory disease. [Method] 90 patients were randomly divided into two groups according to the method of visiting order number by using random parallel control. The control group of 45 cases of levofloxacin 0.3g＋5% glucose 250mL, intravenous drip, 2 times/d, more than 2h. The treatment group of 45 cases of traditional Chinese medicine： Morinda 30g, psoralen, dodder 25g, 20g cinnamon, aconite 5g, eucommia, Atractylodes 20g, 15g yam, ginseng 20g, Decoction 300mL, sooner or later the day; Herbal washing： Xiaoyan soup （honeysuckle, dandelion, Patrinia, Viola 30g, Rhizoma Cyperi, Salvia 15g, safflower 6g, 30g 20g）, Rhizoma Corydalis, Radix Sophorae Flavescentis Decoction 2h, using 300mL, 100mL to 400mL against the warm water wash, 2times/d, 30min/times, pay attention to prevent burns; westernmedicinewiththecontrolgroup. Continuous treatment for 1 courses of 10d. The clinical symptoms, serum inflammatory factors （TNF-, IL-2, IL-4, IL-10） and adverse reactions were observed. 1 treatment courses （10d）, to determine the efficacy. [Results] the treatment group 5 cases were cured, 18 cases markedly effective, 20 cases effective, 2 cases ineffective, the total efficiency of 95.56%; control group 2 cases were cured, 16 cases markedly effective, 19 cases effective, 8 cases ineffective, the total efficiency of 82.22%; the treatment group than the control group （P〈0.05）. TNF-, IL-2, IL-4, IL- l0 two groups were improved （P〈0.01）, the treatment group was better than the control group （P〈0.01）. [Conclusion] Traditional Chinese medicine combined with external washing combined with levofloxacin in the treatment of elderly patients with pelvic inflammatory disease, the effect is satisfactory, no serious adversereactions, worthy of promotion.
