2013-2015年咸水沽医院剖宫产率及剖宫产指征变化分析

Analysis on changes of cesarean section rate and cesarean section indications in Xianshuigu Hospital from 2013 to 2015

目的分析该院近3年来剖宫产率及剖宫产指征的变化,寻找降低剖宫产率的对策。方法选取2013-2015年间在该院生产的7 156例剖宫产产妇的临床资料进行回顾性分析,观察并比较剖宫产率及剖宫产指征的变化情况。结果以瘢痕子宫、产前出血、妊娠期高血压疾病为主要剖宫产指征,各年份组的剖宫产率比较,差异有统计学意义(P<0.05);其次以社会因素、胎儿宫内窘迫为剖宫产指征,各年份组的剖宫产率比较,差异有统计学意义(P<0.05);最后异常分娩、羊水过少、妊娠合并症及多胎妊娠也为剖宫产指征,但各年份组的剖宫产率之间相比,差异不具有统计学意义(P>0.05)。结论社会因素、瘢痕子宫仍是剖宫产率增加的主要原因,提倡阴道试产,严格掌握剖宫产指征,提高产科质量。

Objective To analyze the changes of cesarean section rate and cesarean section indications in Xianshuigu Hospital from2013 to 2015,find out the countermeasures to reduce cesarean section rate. Methods The clinical data of 7 156 delivery women receiving cesarean section in the hospital from 2013 to 2015 were retrospectively analyzed,the changes of cesarean section rate and cesarean section indications were observed and compared. Results Taking scarred uterus,antepartum hemorrhage,and hypertensive disorder complicating pregnancy as the main cesarean section indications,there was statistically significant difference in cesarean section rate among different time groups（ P〈0. 05）; taking social factors and fetal intrauterine distress as the main cesarean section indications,there was statistically significant difference in cesarean section rate among different time groups（ P〈0. 05）; taking abnormal childbirth,oligohydramnios,pregnancy complications,and multiple pregnancy as the main cesarean section indications,there was no statistically significant difference in cesarean section rate among different time groups（ P〉0. 05）. Conclusion Social factors and scarred uterus are still the main reasons for increase of cesarean section rate; promoting vaginal trial production and strictly controlling cesarean section indications can improve obstetric quality.
