糖尿病肾病患者治疗依从性调查分析

Investigation on the Treatment Compliance of Patients with Diabetic Nephropathy

目的通过对糖尿病肾病患者服药依从性和饮食医嘱依从性的调查,分析其可能的影响因素。方法纳入糖尿病肾病患者66例,通过设计问卷调查其服用药物、执行饮食医嘱的依从性,结合患者性别、年龄、文化程度、付费方式、工作状态、参加健康讲座等情况进行分析。结果服药方面,依从性好的患者有43例(65.2%),大学或大学以上学历的患者服药依从性明显高于其它患者(P<0.05);饮食医嘱的依从性方面,能较好执行饮食医嘱的患者有29例(44%),参加过培训班健康讲座的患者其依从性也有高于未参加过培训班健康讲座者的趋势,但差异没有统计学意义(P>0.05)。结论糖尿病肾病患者饮食的依从性普遍较服药依从性差,其中,文化程度可能是此类患者服用药物依从性的一个影响因素,工作的状态、是否参加过饮食培训班也是影响此类患者饮食依从性的因素。所以,加强对患者的针对性健康指导、门诊随访,着重转变糖尿病肾病患者的固有观念,使其改变原有的生活习惯和饮食习惯,提高患者的依从性,保障临床治疗的效果,改善疾病的预后。

Objective Through the investigation on the compliance of the patients with diabetic nephropathy and the compliance of dietary advice, the possible influencing factors were analyzed. Methods 66 cases of patients with diabetic nephropathy were included. The compliance of the medication and diet advice was investigated by questionnaire storey from patient＇s gender, age, education level, the way of payment, the state of work, to participate in health seminars, etc. Results 43 cases （65.2%） had good compliance of the medication, compliance of the medication of patients had college or above degree was significantly higher than that of the other patients （P 〈 0.05） . 29 cases （44%） had diet medication compliance. The patients who participated in the training course had higher compliance than those who did not attend the training class, but the difference was not statistically significant （P 〉 0.05） . Conclusion The dietary compliance of diabetic nephropathy patients is generally poor, and the degree of culture may be one of the factors that affect the compliance of the patients with drug dependence. Therefore, to strengthen the targeted health guidance, follow- up, focus on changing the patient＇s concept, so that it is aware of the importance of diet, living habits in the treatment of diabetic nephropathy, improve patient＇s compliance, guarantee the clinical treatment effect, and improve the prognosis of the disease.
