精神心理障碍对功能性消化不良患者胃功能和生活质量的影响

The influence of Psychological Disorder to the Gastric Function and Quality of Life of Patients with Functional Dyspepsia

目的研究精神心理障碍对功能性消化不良患者的胃功能与生活质量的影响。方法从我院于2014年10月至2016年12月消化内科就诊患者中,选取58例有精神心理障碍的患者为研究组,58例无心理障碍患者为常规组,详细观察比较两组患者生活质量评分(包括情感职能、活力、生理功能、生理职能、社会功能、躯体疼痛、精神健康、总体健康等)、餐前后胃电图、水负荷试验及SPECT胃排空实验等情况。结果常规组患者各项生活质量评分均明显优于研究组(P<0.05);餐前餐后胃电图比较,研究组患者正常节律百分比明显比常规组低(P<0.05),研究组胃动过缓率与主频不稳定系数显著比常规组高(P<0.05);水负荷试验与SPECT胃排空实验比较,常规组患者初始感觉量与最大耐受量明显比研究组更高(P<0.05),而研究组胃排空时间明显长于常规组,差异有统计学意义(P<0.05)。结论精神心理障碍与功能性消化不良密切相关,它可对患者生活质量与胃功能造成一定不良影响。

Objective To study the influence of psychological disorder to the gastric function and quality of life of patients with dyspepsia. Methods 58 cases of patients with psychological disorder were selected from those admitted by the digestive system department of our hospital since October 2014 to December 2016 as research group, and other 58 cases of patients without psychological disorder were selected as regular group. The scores of quality of life, the electrogastrogram before and after meal, water load test and SPECT gastric emptying test between two groups of patients were compared. Results The scoring of quality of life of patients in regular group were dramatically better than those in research group （ P 〈 0.05 ） . For the electrogastrogram before and after meal, the normal rhythm percentage of patients in research group was obviously lower than that in regular group （P 〈 0.05 ） . The bradygastria rate and instability index of dominant frequency of research group were significantly higher than those of regular group （P〈0.05） . For the water load test and SPECT gastric emptying test, the initial sensation quantity and maximum tolerance dose of patients in regular group were dramatically higher than those in research group, and the difference was statistically significant （P 〈 0.05 ） .Condusion The psychological disorder had significant influence on the quality of life and gastric motility function of patients with functional dyspepsia.
