乙型肝炎病毒和丙型肝炎病毒重叠感染患者生化免疫指标检测结果及其临床意义分析

The clinical significance of biochemical and immunological indexes in patients with HBV and HCV overlapping infection

目的分析乙型肝炎病毒(HBV)和丙型肝炎病毒(HCV)重叠感染患者生化免疫指标变化情况,为临床诊治提供依据。方法选择从2014年5月至2016年8月于鄂东医疗集团黄石市中心医院(湖北理工学院附属医院)就诊的96例患者,包括单纯HBV感染患者,单纯HCV感染患者,HBV和HCV重叠感染患者,每组32例患者。观察3组患者主要肝功能指标变化情况,采用酶联免疫吸附试验(ELISA)法检测γ-干扰素(IFN-γ)、IFN-γ诱导蛋白10(IL-10)及炎症因子白细胞介素(IL)-6变化情况。结果3组患者的丙氨酸转氨酶(ALT),天冬氨酸转氨酶(AST)和总胆红素(TBil)比较,差异均有统计学意义(P<0.05),3组患者的ChildPugh评分比较,差异也有统计学意义(P<0.05),3组患者的IFN-γ、IL-10及IL-6比较,差异有统计学意义(P<0.05)。结论 HBV和HCV重叠感染患者肝功能指标变化不甚明显,但是肝脏储备功能下降,可能与加重肝脏的炎症反应有关,对预防重症肝炎的发生有一定的参考价值。

Objective To analyze the clinical significance of biochemical and immunological indexes in pa- tients with HBV and HCV overlapping infection, and to provide the basis for clinical diagnosis and treat- ment. Methods Ninety-six patients were selected from May 2014 to August 2016, including patients with pure HBV infection, HCV infection alone, HBV and HCV overlap infection groups, and 32 cases in each group. The changes of main liver function indexes in three groups were observed, and IFN-γ, IFN-γ induced protein 10 （IL- 10） and inflammatory factor （IL-6） were also measured by ELISA. Results There were significant differences in ALT, AST and TBil among the three groups （ P〈0.05）. There were significant differences in the Child-Pugh score between the three groups （ P d0. 05）, and the difference was statistically significant （ P〈0. 05） . IL-10 and IL-6 levels in the three groups were significantly different （ P〈0.05）. Conclusion Liver function changes in patients with HBV and HCV overlapping infection are not obvious, but the liver reserve function declines, It may be associated with increased inflammation of the liver, which has a certain reference value to the prevention of severe hepatitis.
