右美托咪定对老年胸科手术患者脑氧饱和度及术后认知功能的影响 被引量:10

摘要 目的探讨右美托咪定对老年胸科手术患者脑氧饱和度[Sct(O2)]和术后认知功能的影响。方法选取98例老年胸科手术患者,将其随机分为观察组和对照组各49例,观察组于切皮前15 min内泵注1μg/kg右美托咪定,随后以0.3~0.5μg/(kg·h)速度持续泵注至手术结束前30 min停药;对照组则不用右美托咪定。术中均以异丙酚-瑞芬太尼持续泵注、罗库溴铵间断注射维持麻醉。监测Sct(O2),记录诱导前(t0)、诱导后(t1)、单肺通气后30min(t2)、术毕(t3)以及苏醒后(t4)的Sct(O2);应用简易智力状态检查(MMSE)评估患者术前1 d和术后6,12,24 h的认知功能变化。结果 t2时观察组Sct(O2)水平显著高于对照组(P<0.05);对照组在t2时Sct(O2)水平明显降低,与其他时间点相比差异有统计学意义(P<0.05)。与术前1 d相比,2组术后6,12 h的MMSE评分均明显降低(P均<0.05);术后6 h、12 h观察组认知功能障碍发生率均明显高于对照组(P<0.05);术后24 h 2组患者认知功能均恢复正常。结论右美托咪定可减轻老年胸科手术患者单肺通气期间Sct(O2)下降,进而改善术后认知功能,降低POCD的发生率。
