不同妊娠次数O型血孕妇新生儿ABO溶血病发生情况及其与体内抗体效价的相关性研究

A Study on Incidence of ABO Haemorrhagic Disease of Newborn Gaving Birth by O-type Blood Puerpera Women with Different Gravidities and Its Relationship with Antibody Titer in Vivo

目的探讨不同妊娠次数O型血孕妇新生儿ABO溶血病(ABO-HDN)发生情况及其与体内抗体效价的相关性。方法选择2013年1月—2016年5月86例首次妊娠(A组)、48例2次妊娠(B组)和58例>2次妊娠的O型血孕妇,观察新生儿ABO-HDN的发生情况,并行血清IgG抗体效价检测,分析二者的相关性。结果 B组和C组IgG抗A(B)效价总阳性率高于A组,且C组高于B组(P<0.05)。随着IgG抗A(B)效价的升高,新生儿ABO-HDN发病率呈上升趋势(P<0.05)。妊娠次数与IgG抗A(B)效价及新生儿ABO-HDN发病率呈正相关(P<0.05)。结论O型血孕妇产前血清IgG抗A(B)效价检测可减少或避免新生儿ABO-HDN的发生。

Objective To investigate incidence of ABO-hemolytie disease of newborn （ABO-HDN） giving birth by O-type blood puerperal women with different gravidities and its correlation with antibody titer in vivo. Methods A to- tal of 86 O-type blood women with primigravida （group A） , 48 O-type blood women with two times of pregnancies （group B） and 58 O-type blood women with more than 2 times of pregnancies （group C） during January 2013 and May 2016 were recruited in this study. Incidence of ABO-HDN was observed, and serum IgG antibody titer detection was also per-formed, and the relationship between incidence of ABO-HDN in O-type blood puerperal women with different gravidities and its antibody titers in vivo was analyzed. Results The total positive rates of IgG anti-A （B） antibody titer in group B and C were significantly higher than that in group A, and the rate in group C was significant higher than that in group B （ P 〈0. 05 ）. Incidence rate of ABO-HDN was increased with the increasing IgG anti-A （B） antibody titer （ P 〈 0. 05 ）. Gravidity was positively related to IgG anti-A （B） antibody titer and ABO-HDN morbidity （P 〈 0. 05）. Conclusion Prenatal serum IgG anti-A （B） antibody titer detection can reduce or avoid the incidence of ABO-HDN for O-type blood puerperal women.
