膝关节镜术后关节腔注射不同浓度右美托咪定复合罗哌卡因的镇痛效果 被引量:6

摘要 目的观察关节腔内注射右美托咪定复合罗哌卡因对膝关节镜术后镇痛的效果。方法 60例择期膝关节镜手术患者随机分为A、B、C 3组,术毕A组关节腔内注射0.25%罗哌卡因20 ml,B组关节腔内注射0.25%罗哌卡因+0.5μg/kg右美托咪定20 ml,C组0.25%罗哌卡因+1μg/kg右美托咪定20 ml,视觉模拟评分(VAS)≥4分时肌注芬太尼镇痛,镇痛效果根据VAS、镇痛持续时间和术后24 h内芬太尼用量评估。结果术后B、C组VAS及芬太尼用量显著低于A组,C组VAS及芬太尼用量显著低于B组(P<0.05)。B、C组术后镇痛持续时间显著长于A组,C组术后镇痛持续时间显著长于B组(P<0.05)。结论关节腔内注射右美托咪定可明显增强罗哌卡因用于膝关节镜术后镇痛的效果,且其镇痛效果呈浓度依赖性。
