网络互动式连续护理干预对妊娠期糖尿病患者生存质量的影响

Influence of network interactive continuous nursing intervention on life quality of patients with gestational diabetes mellitus

目的:探讨网络互动式连续护理在改善妊娠期糖尿病(GDM)患者自我管理能力与生存质量中的应用价值。方法:将自愿接受本研究的GDM初产妇共172例,随机分为对照组和干预组各86例。对照组采用常规护理方法;干预组在常规护理基础上实施网络互动式连续护理干预方案,建立GDM患者电子健康档案,借助GDM患者微信交流群,应用健康宣教与心理干预、饮食干预、适度运动干预、孕期与产后血糖监测、胰岛素治疗等干预。对出院后的患者定期随访实现全程连续护理。比较两组GDM患者的生存质量及自我管理水平的差异。结果:通过实施网络互动式连续护理干预,在干预组患者生存质量评分问卷中各影响维度中,生存质量总分、对工作及前途的影响、身体健康状况自评评分均高于对照组,差异有统计学意义(P<0.05);干预组患者自我管理态度与能力、运动依从性、饮食依从性,均高于对照组,差异有统计学意义(P<0.05)。血糖和尿糖监测方面两组差异无统计学意义(P>0.05)。结论:网络互动式连续护理,可有效加强妊娠期糖尿病患者的自我管理水平,提高患者的生存质量。

Objective To explore the application value of network interactive continuous care in improving self-management ability and quality of life in patients with gestational diabetes mellitus （GDM）, and to provide reliable clinical evidence for continuous nursing appli-cation of gestational diabetes. Methods： A total of 172 primiparas with GDM who had voluntarily accepted the study were randomly as-signed to the control group and the intervention group, 86 cases in each group. Control group received routine nursing care, intervention group received continuous network interactive nursing, which included establishing electronic health records, health education and psycho-logical intervention, dietary intervention, moderate exercise intervention, monitoring blood glucose during pregnancy and postpartum, fol-lowing up regularly after discharge. The level of self management and life quality were compared between two groups. Results： The score of life quality, the impact on jobs and the future, health self critiquing points in intervention group were higher than the control group at 42 days after delivery （P〈0.05）. The self management ability, exercise compliance, dietary compliance were also higher than the control group （P〈0.05）. Conclusion： Network interactive continuous care can effectively improve the self-management of patients with GDM, improve the life quality of them.
